目的探讨甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原125(CAl25)和糖类抗原199(CA199)测定对肝癌的诊断价值。方法选取2012年6月间至2014年6月河南省商丘市第一人民医院收治的120例肝癌患者,根据肿瘤病理分型分为观察A组(肝细胞癌患者60例)和观察B组(肝内胆管癌患者60例)。选取同期120例健康体检者作为健康对照组。采用化学发光方法检测肝癌患者及健康体检者血清中AFP、CEA、CAl25和CAl99的水平。结果肝癌患者血清中AFP、CEA、CAl25和CA199水平较健康体检者升高,差异有统计学意义(P<0.05)。观察A组患者血清中AFP水平升高较观察B组明显,观察B组患者血清中CEA、CAl25和CAl99水平升高较观察A组明显,差异均有统计学意义(均P<0.01)。结论四种肿瘤标志物中,AFP诊断肝细胞癌的价值较高,CEA、CAl25和CAl99诊断肝内胆管细胞癌价值较高。联合多项肿瘤标志物检测,可以提高肝肿瘤的早期诊断率。
Objective To investigate the diagnostic value of AFP, CEA, CA125 and CA199 in the diagnosis of liver cancer. Methods A total of 120 HCC patients admitted to the First People’s Hospital of Shangqiu City, Henan Province from June 2012 to June 2014 were divided into observation group A (hepatocellular carcinoma 60 patients) and observation group B Intrahepatic cholangiocarcinoma in 60 patients). Select the same period 120 cases of healthy people as a healthy control group. The levels of AFP, CEA, CA125 and CAl99 in serum of hepatocellular carcinoma patients and healthy subjects were detected by chemiluminescence method. Results The levels of serum AFP, CEA, CA125 and CA199 in patients with HCC were significantly higher than those in healthy controls (P <0.05). The level of serum AFP in group A was significantly higher than that in group B, and the level of CEA, CA125 and CAl99 in group B was significantly higher than that in group A (all P <0.01). Conclusion The diagnostic value of AFP in hepatocellular carcinoma is higher among the four tumor markers. CEA, CA125 and CA199 are more valuable in diagnosing intrahepatic cholangiocarcinoma. Joint detection of a number of tumor markers, can improve the early diagnosis of liver tumors.
