目的探讨99mTc-DTPA肾动态显像对重度肾积水患者患肾功能评估的意义。方法 63例重度肾积水患者行99Tcm-DTPA肾动态显像评估其肾功能,并与IVP结果对照,按其肾功能结果决定手术方式,并于术后1年内复查。结果 63例患者有重度积水肾69个,其中无功能肾15个,功能严重受损肾21个,肾功能中度受损16个,肾功能轻度受损12个,肾功能正常4个,切除患肾13个,其他50例患肾行手术解除梗阻,43例术后1年内复查肾动态显像,肾功能轻中度受损者术后肾功能恢复良好,肾功能严重受损者肾功能未见明显恢复。结论 99mTc-DTPA肾动态显像对重度肾积水患者手术前评估肾功能有重要意义。 Objective To investigate the significance of 99mTc-DTPA renal dynamic imaging in assessing renal function in patients with severe hydronephrosis. Methods Sixty-three patients with severe hydronephrosis were evaluated with 99Tcm-DTPA renal dynamic imaging to evaluate the renal function. The renal function was compared with the results of IVP. The operative methods were determined according to the results of renal function and were reviewed within 1 year after operation. Results Sixty-three patients had 69 patients with severe hydronephrosis including 15 non-functioning kidney, 21 with severely impaired function, 16 moderately impaired renal function, 12 impaired renal function and 4 normal renal function , 13 cases of diseased kidneys were removed and the other 50 cases were treated with renal surgery to lift the obstruction. Renal dynamic imaging was performed in 43 cases within 1 year after surgery. Renal function was well recovered and renal function was severely impaired No significant recovery of renal function. Conclusion 99mTc-DTPA renal dynamic imaging in patients with severe hydronephrosis before surgery to assess renal function is of great significance.
