【目的】观察持续正压通气对重度支气管哮喘患儿肺功能的影响。【方法】将重度支气管哮喘患儿168例随机分为治疗组[持续正压通气(continious positive airway pressure,CPAP)]和对照组(面罩吸氧),两组均给予常规治疗。3d后监测喘息缓解情况、哮鸣音消失时间、最大呼气峰流速(PEFR)、第1秒最大呼气率(FEV1%)、用力肺活量(FVC)、血气分析(PaO2)等指标的变化。【结果】治疗组在哮喘缓解时间、哮鸣音消失时间、PEFR、FEV1%、PaO2等方面与对照组间差异有统计学意义(P<0.05)。【结论】持续正压通气可以明显改善重度支气管哮喘患儿的症状和肺功能。
【Objective】 To observe the effect of continuous positive pressure ventilation on pulmonary function in children with severe bronchial asthma. 【Methods】 One hundred and eighty-eight children with severe bronchial asthma were randomly divided into treatment group [continual positive airway pressure (CPAP)] and control group (mask oxygenation). Both groups were given routine treatment. After 3 days, the changes of wheeze relief time, wheeze disappearance time, maximum expiratory flow rate (PEFR), maximum expiratory volume in one second (FEV1%), forced vital capacity (FVC) and blood gas analysis (PaO2) were monitored. 【Results】 There was significant difference between the treatment group and the control group in terms of asthma relief time, disappearance of wheeze, PEFR, FEV1% and PaO2 (P <0.05). 【Conclusion】 Continuous positive pressure ventilation can significantly improve the symptoms and pulmonary function in children with severe bronchial asthma.
