目的:对我院2011年到2013年的门诊处方进行点评评析,整体性掌握用药基本情况,通过处方点评指标促进处方质量的提高,同时改善临床用药的有效、安全、合理。方法:随机抽取我院在2011年到2013年的门诊处方,其中从2011年每月已归档的处方中随机抽取100份门诊处方(全年共1200份);随机抽取2012年和2013年每天的门诊处方(共20份),两年共计14500份。结果:在所选取的14500份处方当中,2011年到2013年的处方合格率分别为:2011年91.5%,2012年98.1%,2013年99.5%。结论:尽管我院处方点评指标控制合理,但临床仍存在个别用药不合理现象,但通过对《处方管理办法》,《抗菌药物管理办法》等相关法律法规的有力执行,我院的门诊处方质量已表现出明显改善;今后工作还应在药剂科工作职能的强化、药师与医生配合机制等方面进一步强化。
OBJECTIVE: To review and comment outpatient prescriptions from 2011 to 2013 in our hospital, to master the basic situation of medication, to promote the improvement of prescription quality through prescription review indicators, and to improve the effectiveness, safety and rationality of clinical medication. Methods: Outpatient prescriptions were randomly selected in our hospital from 2011 to 2013, of which 100 outpatient prescriptions were randomly selected from a total of 1,200 prescriptions filed each month from 2011 to 2011; Outpatient prescriptions (a total of 20), a total of 14,500 for two years. Results: Among the 14,500 prescriptions selected, the pass rates for prescriptions from 2011 to 2013 were 91.5% in 2011, 98.1% in 2012 and 99.5% in 2013. CONCLUSIONS: Although the control index of prescriptions in our hospital is reasonable, there are still some unreasonable individual medication in clinical practice. However, through the effective implementation of the relevant laws and regulations such as “prescription management approach” and “antibacterial drug management measures”, the outpatient prescription quality Has shown a clear improvement; the future work should be further strengthened in the pharmacy department’s job functions, pharmacists and doctors with the mechanism and so on.
