目的通过分析腰椎骨髓脂肪含量与年龄、性别、体重指数(BMI)及腰围的相关性,探讨骨髓脂肪含量的体质特点,以加深对骨质疏松发生规律的认识。资料与方法纳入健康志愿者或慢性腰腿痛患者144例,测量其身高、体重、腰围,并计算BMI。采用单体素点分辨波谱法对L3椎体进行波谱采集,以1.3 ppm波峰为三酰甘油峰,水峰位于4.67 ppm,计算脂水比与脂肪比例。比较不同性别、年龄、BMI、腰围组间L3椎体脂水比、脂肪比例的差异。分析年龄与L3椎体脂水比及脂肪比例的相关性。结果男、女L3椎体脂水比、脂肪比例总平均值比较差异无统计学意义(t=-0.267、-0.993,P>0.05)。不同年龄段患者L3椎体脂水比、脂肪比例的总平均值差异有统计学意义(F=3.723、5.478,P<0.05)。女性61~70岁组L3椎体脂水比、脂肪比例均明显高于20~30岁、31~40岁、41~50岁、51~60岁组(P<0.05),女性L3椎体脂肪比例20~30岁组低于>70岁组(P<0.05)。61~70岁组男女脂水比总平均值均明显高于20~30岁、31~40岁、41~50岁、51~60岁组(P<0.05)。20~30岁组男女脂肪比例总平均值均低于31~40岁、41~50岁、51~60岁、61~70岁、>70岁组(P<0.05),61~70岁组与31~40岁、41~50岁、51~60岁组比较,差异均有统计学意义(P<0.05)。不同BMI及男性和女性不同腰围组间L3椎体脂水比及脂肪比例比较,差异均无统计学意义(P>0.05)。年龄与L3椎体脂肪比例呈正相关(r=0.321,P<0.05)。结论腰椎骨质脂肪含量与患者年龄相关,与性别、BMI、腰围均无关。
OBJECTIVE: To investigate the relationship between the fat content of lumbar marrow and age, sex, body mass index (BMI) and waist circumference, and to explore the physical characteristics of bone marrow fat content to deepen the understanding of the occurrence of osteoporosis. Materials and Methods 144 healthy volunteers or patients with chronic low back pain were included in this study. Their height, weight and waist circumference were measured and BMI was calculated. The L3 vertebral body was collected by single-element point-resolved spectroscopy. The triglyceride peak at 1.3 ppm and the water peak at 4.67 ppm were used to calculate the lipid-to-fat ratio. The differences of fat-to-fat ratio and fat percentage in L3 vertebral body between different gender, age, BMI and waist circumference were compared. Analysis of age and L3 vertebral fat ratio and fat correlation. Results There were no significant differences in fat-to-water ratio and fat percentage between male and female L3 vertebra (t = -0.267, -0.993, P> 0.05). There were significant differences in the total average of fat-to-fat ratio of L3 vertebral body in patients of different ages (F = 3.723,5.478, P <0.05). The lipid-water ratio and fat percentage of L3 vertebral body in 61-70-year-old women were significantly higher than those of 20-30 years old, 31-40 years old, 41-50 years old, 51-60 years old women (P <0.05) The proportion of 20 ~ 30 years old group was lower than> 70 years old group (P <0.05). The total lipid-to-water ratio of men and women in 61- to 70-year-old group was significantly higher than that of 20-30 years old, 31-40 years old, 41-50 years old, 51-60 years old group (P <0.05). The average percentage of male and female fat in 20-30 years old group was lower than that of 31-40 years old, 41-50 years old, 51-60 years old, 61-70 years old,> 70 years old group (P <0.05) The difference was statistically significant between 31-40 years old, 41-50 years old and 51-60 years old group (P <0.05). There was no significant difference in fat-to-water ratio and fat percentage in L3 between different BMI groups and different waist circumference groups (P> 0.05). There was a positive correlation between age and L3 vertebral body fat (r = 0.321, P <0.05). Conclusions The lumbar bone fat content is related to patient’s age, but not to sex, BMI and waist circumference.
