目的观察止痛健骨方对兔膝骨关节炎软骨MMP-1的影响及与MRI分级的相关性分析。方法将90只白兔随机分为正常对照组、模型对照组、阳性药物组、止痛健骨低剂量组、止痛健骨高剂量组,每组各18只,采用木瓜蛋白酶制备兔膝骨关节炎模型,止痛健骨低剂量组、止痛健骨高剂量组分别给与止痛健骨方低剂量、高剂量药物灌胃,阳性药物组给与盐酸氨基葡萄糖片灌胃,0.0672g/kg,正常对照组和模型对照组则给与生理盐水灌胃。在模型验证、实验灌胃第4周、第8周三个时间点,各组随机选择6只白兔,进行膝关节MRI扫描,并检测软骨MMP-1表达水平,进行对比比较。结果在实验干预前、实验干预第4周、第8周,与正常对照组比较,模型对照组、阳性药物组、止痛健骨低剂量组、止痛健骨高剂量组软骨MMP-1表达、MRI软骨分级均上升,差异有统计学意义(P<0.05)。与模型对照组比较,阳性药物组、止痛健骨低剂量组、止痛健骨高剂量组第4周、第8周MMP-1表达下降,差异有统计学意义(P<0.05)。采用等级Spearman等级相关分析发现软骨MMP-1表达增加,对应软骨MRI分级也上升,两者呈直线正相关(r=0.815,P=0.000)。结论止痛健骨方能降低兔KOA模型关节软骨MMP-1表达,降低软骨MRI分级,且两者成正向直线相关,这可能为其治疗KOA的作用机理。
Objective To observe the effect of Zhitong Jiangu Recipe on the expression of MMP-1 in rabbit knee osteoarthritis cartilage and its correlation with MRI grading. Methods Ninety rabbits were randomly divided into normal control group, model control group, positive drug group, low-dose analgesic-bone-setting group and high-dose analgesic-bone group, 18 rats in each group. Papain was used to prepare rabbit knee osteoarthritis The model group, the low-dose pain-relieving bone-setting group and the high-dose pain-relieving bone-adding group were given low-dose and high-dose medicine for pain-relieving and gouty-bone-killing respectively and the positive drug group was given intragastric administration of glucosamine hydrochloride (0.0672g / Group and model control group was given saline gavage. At the 4th and 8th week after model validation, three rabbits were randomly selected in each group for knee MRI scanning. The expression of MMP-1 in cartilage was detected and compared. Results Compared with the normal control group, the expression of MMP-1 in the cartilage of model control group, positive drug group, low-dose analgesic-bone-bearing group and high-dose analgesic-bone- Cartilage grade were increased, the difference was statistically significant (P <0.05). Compared with the model control group, the expression of MMP-1 in the positive drug group, the low-dose analgesic-bone-bearing group and the high-dose analgesic-bone-bearing group at the 4th and the 8th week were significantly decreased (P <0.05). Spearman rank correlation analysis showed that cartilage MMP-1 expression increased corresponding cartilage MRI grade also increased, the two were linearly correlated (r = 0.815, P = 0.000). Conclusion Zhiganjiangyu decoction can decrease the expression of MMP-1 in articular cartilage and decrease the grade of cartilage MRI in rabbits with KOA model, and the positive correlation between them can be attributed to the mechanism of KOA.
