目的探讨椎动脉超声应用多探头联合检测对椎基底动脉供血不足(VBI)的诊断价值。方法 89例VBI患者(VBI组),其中男性49例,女性40例,平均年龄58.0岁;40例正常人(对照组),其中男性21例,女性19例,平均年龄53.0岁。采用小凸阵探头(6.0~10.0 MHz)、线阵探头(5.0~10.0 MHz)及凸阵探头(2.0 MHz)对VBI组和对照组患者的椎动脉起始段、颈段及颅内段的走行、结构和血流动力学参数进行检测分析。结果高频小凸阵探头诊断VBI的灵敏度为65.6%;高频线阵探头诊断VBI灵敏度为81.8%;低频凸阵探头诊断VBI灵敏度为89.2%;三者联合检查,灵敏度达94.2%。结论多探头联合应用检查椎动脉,可以较全面观察椎动脉的走行和结构,获得血流动力学参数,提高超声对VBI诊断的灵敏度。
Objective To investigate the diagnostic value of multi-probe combined with vertebral artery sonography in the diagnosis of vertebrobasilar insufficiency (VBI). Methods A total of 89 VBI patients (VBI group), including 49 males and 40 females, with an average age of 58.0 years and 40 normal controls (control group), including 21 males and 19 females with a mean age of 53.0 years. In the VBI group and the control group, the vertebral artery in the initial, the cervical and the intracranial segments of the vertebral artery were examined with a small convex array probe (6.0-10.0 MHz), a linear array probe (5.0-10.0 MHz) and a convex array probe (2.0 MHz) Walk, structure and hemodynamic parameters for detection analysis. Results The sensitivity of VBI was 65.6%. The sensitivity of high frequency linear array probe was 81.8%. The sensitivity of low frequency convex array probe was 89.2%. The sensitivity of VBI was 94.2%. Conclusion Combined multi-probe examination of the vertebral artery can be more comprehensive observation of the vertebral artery walk and structure, to obtain hemodynamic parameters, to improve the sensitivity of ultrasonic diagnosis of VBI.
