目的研究肝癌患者外周血中T淋巴细胞表面标志潜态相关性肽链(LAP)的表达特点以及肝癌根治性切除术对LAP表达的影响,探讨其在肝癌发生发展中的作用。方法抽取北京大学深圳医院2011年10月到2012年12月期间30例肝癌患者的外周静脉血与30名健康志愿者的外周静脉血,采用流式细胞术检测LAP+CD4+T细胞在肝癌患者和正常人外周血单个核细胞中的比例,并检测LAP+CD4+T细胞在肝癌患者术前和术后的外周血单个核细胞中的比例。结果 LAP+CD4+T细胞在肝癌患者外周血中的表达水平明显高于正常组(1.84±0.85 vs.0.73±0.39,P=0.006),差异有统计学意义。肝癌患者外周血中LAP+CD4+T细胞术后组比术前组显著下降(1.07±0.35 vs.1.84±0.85,P=0.001),差异有统计学意义。肝癌术后组和正常组相比,LAP+CD4+T细胞略微有所升高,但差异无统计学意义(1.07±0.35 vs.0.73±0.39,P=0.209)。结论 LAP+CD4+T细胞在肝癌患者外周血中的表达比例明显上升,肝癌根治性切除术后患者外周血中的LAP+CD4+T细胞的比例较术前显著下降并接近正常水平,提示肝癌患者外周血中高水平的具有抑制效应的LAP+CD4+T细胞可能是肝癌局部微环境中负性调节机制的重要组成,在肝癌免疫逃逸中发挥积极作用。
Objective To investigate the expression of TAP-related peptide (LAP) in peripheral blood of patients with hepatocellular carcinoma (HCC) and the effect of radical resection on the expression of LAP in hepatocellular carcinoma (HCC) and to explore its role in the development of hepatocellular carcinoma. Methods Peripheral venous blood was collected from peripheral blood of 30 HCC patients and 30 healthy volunteers from Peking University Shenzhen Hospital from October 2011 to December 2012. Flow cytometry was used to detect the expression of LAP + CD4 + T cells in HCC patients And normal human peripheral blood mononuclear cells, and to detect the proportion of LAP + CD4 + T cells in peripheral blood mononuclear cells of preoperative and postoperative liver cancer patients. Results The expression level of LAP + CD4 + T cells in peripheral blood of patients with hepatocellular carcinoma was significantly higher than that of the normal group (1.84 ± 0.85 vs.0.73 ± 0.39, P = 0.006), the difference was statistically significant. The levels of LAP + CD4 + T cells in peripheral blood of HCC patients were significantly lower than those before operation (1.07 ± 0.35 vs.1.84 ± 0.85, P = 0.001). The difference was statistically significant. Compared with the normal group, LAP + CD4 + T cells increased slightly in the postoperative group, but the difference was not statistically significant (1.07 ± 0.35 vs.0.73 ± 0.39, P = 0.209). Conclusions The proportion of LAP + CD4 + T cells in peripheral blood of patients with hepatocellular carcinoma was significantly increased. The proportion of LAP + CD4 + T cells in peripheral blood of patients with hepatocellular carcinoma after radical resection was significantly lower than that before operation, suggesting that liver cancer High levels of inhibitory effector LAP + CD4 + T cells in peripheral blood of patients may be an important component of the negative regulatory mechanism in the local microenvironment of HCC and play an active role in the immune escape of HCC.
