高钾血症型肾小管酸中毒(RTA)是一种常见的RTA。本文报告22例高钾血症型RTA,并测定了患者血浆肾素(PRA)、血管紧张素-Ⅱ(AT-Ⅱ)和醛固酮水平,结果表明:(1)该型RTA在慢性肾小球肾炎和慢性间质性肾炎中最为多见(2)低醛固酮血;症或(和)肾小管对醛固酮反应降低,可能是该型RTA发病的主要原因;(3)低肾素血症是醛固酮不足的主要原因;(4)低醛固酮血症与肾功能损害无关,但后者可能是该型RTA的易患因素之一。该型RTA的发病机理和发生率尚需进一步研究。
Hyperkalemia type renal tubular acidosis (RTA) is a common RTA. In this paper, 22 cases of hyperkalemia type RTA were reported and the levels of plasma renin (PRA), angiotensin-Ⅱ (AT-Ⅱ) and aldosterone were measured. The results showed that: (1) Nephritis and chronic interstitial nephritis, the most common (2) low aldosterone blood; disease or (and) renal tubular aldosterone reaction decreased may be the main cause of this type of RTA; (3) low renin hyperlipidemia is aldosterone Lack of the main reason; (4) low aldosteronism and renal dysfunction has nothing to do, but the latter may be one of the risk factors for this type of RTA. The pathogenesis and incidence of this type of RTA still need further study.
