目的 探讨上气道二维影像替代三维影像的可能性。方法 1 6名阻塞性睡眠呼吸暂停综合征 ( Obstructive Sleep Apnea Syndrom e,OSAS)患者摄制上气道的磁共振 ( Magnetic Resonance Imag-ing,MRI)片及仰卧位 X线头颅侧位片 ,2 3名 OSAS患者摄制 MRI及直立位 X线片 ,分别对二者两种影像资料中的上气道及软腭、舌体进行比较。结果 上气道阻塞点距腭平面的距离在二者的比较中 ,均无统计学差异。在上气道大小的比较中 ,软腭、舌体的 MRI测量值与 X线测量值相关性较好 ( P<0 .0 5 ) ;仰卧在 X线片在腭咽、舌咽 MRI与 X线指标的二者相关程度较好 ( P<0 .0 5 )直立位则在喉咽的 MRI与 X线符合程度上较好 ( P<0 .0 5 )。但 X线对于阻塞严重程度的分组与 MRI出入较大。结论 在有关上气道的研究中 ,二维影像手段不能完全代替三维影像手段 ;但二维影像能较好地反映软腭、舌体的大小 ,能揭示阻塞的部位 ,能在一定程度上反映腭咽、舌咽、喉咽的大小。仰卧位 X线片优于直立位 X线片。
Objective To explore the possibility of replacing the three-dimensional images by two-dimensional images of the upper airway. Methods Sixteen patients with obstructive sleep apnea syndrome (OSAS) were involved in the magnetic resonance imaging (MRI) of the upper airway and the supine X-ray cephalometric slices. One patient with OSAS filming MRI and upright X-ray film, respectively, the two images in the two upper airway and soft palate, tongue body were compared. Results The distance between upper airway obstruction point and palatal plane was not statistically different between the two. In the comparison of the size of the upper airway, the soft palate, the tongue of the MRI measurement value and the X-ray measured value is better (P <0. 05); supine on the X-ray in the velopharyngeal, glossopharyngeal MRI and X-ray The correlation between the two indexes was good (P <0.05). The upright position was better in the coincidence of MRI and hypopharynx (P <0.05). However, the X-ray grouping and MRI for the severity of blockage greater access. Conclusions In the study of the upper airway, the two-dimensional imaging method can not completely replace the three-dimensional imaging means; however, the two-dimensional image can better reflect the size of the soft palate and the tongue body, reveal the obstructed site and can reflect the palate to a certain extent Pharynx, glossopharyngeal, hypopharyngeal size. Supine X-ray film is superior to the vertical X-ray film.
