目的探讨持续子宫按摩在减少产后出血、防止宫腔积血中的作用。方法选择2008年1月—2012年1月收治的具有产后出血高危因素(巨大儿、多胎妊娠、羊水过多、滞产、子痫前期)的孕产妇800例,随机分为对照组和实验组各400例,对照组采用常规预防产后出血方法,实验组在对照组基础上,于胎盘娩出后持续按摩子宫30 min,比较两组产后出血发生率及产后24 h平均出血量。结果产后出血发生率对照组5.75%,实验组1.00%,两组比较差异有统计学意义(P<0.05);平均出血量对照组(470±20)ml,实验组(340±23)ml,两组比较差异有统计学意义(P<0.05)。结论持续子宫按摩可以明显减少产后出血量,降低产后出血发生率,是预防具有高危因素产后出血的一种简便、安全、有效的方法,值得临床推广应用。
Objective To investigate the effect of continuous uterine massage on reducing postpartum hemorrhage and preventing uterine hemorrhage. Methods A total of 800 pregnant women with high risk of postpartum hemorrhage (macrosomia, multiple pregnancy, polyhydramnios, prolonged labor, preeclampsia) who were admitted from January 2008 to January 2012 were randomly divided into control group and experimental group 400 cases in each group. The control group was treated with conventional methods of preventing postpartum hemorrhage. The experimental group continued to massage the uterus for 30 minutes after the placenta was delivered on the basis of the control group. The incidence of postpartum hemorrhage and the average amount of bleeding at 24 hours postpartum were compared between the two groups. Results The incidence of postpartum hemorrhage was 5.75% in the control group and 1.00% in the experimental group, with significant difference between the two groups (P <0.05). The average amount of bleeding in the control group (470 ± 20) ml, experimental group (340 ± 23) The difference between the two groups was statistically significant (P <0.05). Conclusion Continuous uterine massage can significantly reduce the amount of postpartum hemorrhage and reduce the incidence of postpartum hemorrhage. It is a simple, safe and effective method to prevent postpartum hemorrhage with high risk factors and is worthy of clinical application.
