目的探讨职业性慢性苯中毒的临床特点、治疗与转归。方法分析49例职业性慢性苯中毒患者中毒程度与工龄、外周血细胞计数间的关系,以及治疗前后症状、外周血细胞计数的差异。结果 49例职业性慢性苯中毒患者中37例轻度中毒,6例中度中毒,6例重度中毒。轻度组、中度组、重度组的白细胞、中性粒细胞、红细胞、血红蛋白、血小板计数均低于对照组,且差异有统计学意义(均P<0.05);轻度组、中度组与重度组间红细胞计数结果差异有统计学意义(P<0.05),轻度组与重度组间血红蛋白、血小板计数差异有统计学意义(P<0.05);患者3组间白细胞、中性粒细胞计数差异无统计学意义(P>0.05)。职业性慢性苯中毒患者经治疗后,头晕、乏力、失眠、多梦、牙龈出血、皮下瘀斑症状均有不同程度减轻;治疗前后白细胞、中性粒细胞、红细胞、血红蛋白、血小板计数差异均无统计学意义(P>0.05)。结论职业性慢性苯中毒患者红细胞、血红蛋白、血小板计数有随苯中毒轻、中、重依次递减趋势;目前治疗可缓解一般症状,但较难改善外周血白细胞、中性粒细胞、红细胞、血红蛋白等指标。
Objective To investigate the clinical features, treatment and prognosis of occupational chronic benzene poisoning. Methods 49 cases of chronic occupational benzene poisoning poisoning degree and length of service, the relationship between the number of peripheral blood cells, as well as before and after treatment of symptoms, peripheral blood cell count differences. Results Among 49 cases of chronic benzene poisoning, 37 were moderately poisoned, 6 were moderately poisoned and 6 were severe poisoned. The counts of leucocyte, neutrophil, erythrocyte, hemoglobin and platelet in mild group, moderate group and severe group were lower than those in control group (all P <0.05); mild and moderate group (P <0.05). There was significant difference of hemoglobin and platelet count between mild group and severe group (P <0.05). There was no significant difference between the three groups in the number of leucocyte, neutrophil No significant difference in count (P> 0.05). After treatment, the symptoms of dizziness, fatigue, insomnia, multiple dreams, gingival bleeding and subcutaneous ecchymosis were all relieved in patients with occupational chronic benzene poisoning. Before and after treatment, there were no differences in leucocyte, neutrophil, erythrocyte, hemoglobin and platelet count Statistical significance (P> 0.05). Conclusions Red blood cells, hemoglobin and platelet count of patients with occupational chronic benzene poisoning tend to decrease with light, medium and heavy as benzene poisoning. The current treatment can relieve the general symptoms, but it is difficult to improve the peripheral blood leucocytes, neutrophils, red blood cells, hemoglobin, etc. index.
