下颌种植覆盖义齿不同类型附着体的固位力衰减研究

目的:对比研究下颌种植覆盖义齿3种不同类型附着体的固位力衰减趋势。方法:纳入三型附着体试件。A组:钛球/钛帽附着体;B组:钛杆/钛卡附着体;C组:磁性附着体。制作标准下颌种植覆盖义齿实验模型,分别连接各型附着体。对所有试件进行固位力测试,分别行初始固位力测试,固位力衰减测试(共脱位5000次,每脱位40次重复测试3次固位力数值),终末固位力测试。并观察三种类型附着体的固位力衰减趋势。结果:经5000次摘戴循环后,A组固位力衰减(22.82±0.07)N,B组衰减(11.25±0.05)N,C组衰减(0.02±0.001)N。统计分析表明:初始固位力三组间均存在统计学差异,A组初始固位力最大,C组初始固位力最小;终末固位力三组间均存在统计学差异,B组终末固位力最大,C组终末固位力最小;三组衰减趋势的差异均存在统计学意义,其中C组固位力衰减趋势最小,A组固位力衰减趋势最大。结论:①三组附着体的固位力均随摘戴次数的增加而逐渐降低。②钛球/钛帽附着体的初始固位性能最佳,固位力衰减趋势最大。③磁性附着体初始及终末固位力均为三组中最差。④钛杆/钛卡附着体终末固位力三组中最佳,能维持较好修复效果。 OBJECTIVE: To compare the declining trend of retention of three different types of attachment with mandibular implant overdenture. Methods: Three types of attachment specimens were included. Group A: titanium ball / titanium cap attachment; Group B: titanium rod / titanium card attachment; Group C: magnetic attachment. Standard mandibular implant overdenture experimental model, respectively, to connect various types of attachment. All the specimens were tested for retentive force, initial retentive force test, retention decay test (total dislocation 5000 times, dislocation 40 times repeated test 3 times retentive value), final retention force test. And observe the retention of three types of attachment decay trend. Results: After 5000 cycles of reattachment, the retention of A in group A decayed (22.82 ± 0.07) N, in group B decayed (11.25 ± 0.05) N, and in group C decayed (0.02 ± 0.001) N. Statistical analysis showed that there were statistical differences among the three groups, the initial retentive force in group A was the highest, and the initial retentive force in group C was the lowest. There were significant differences among the three groups The maximum retention strength at the end and the smallest retention strength at the terminal of Group C; the decay tendency of the three groups showed statistically significant differences, of which the decay tendency of C group was the smallest, and the decay tendency of Group A was the largest. Conclusion: (1) The retention force of three groups of attachment gradually decreases with the increase of the number of wearing. ② The initial retention of the titanium ball / titanium cap attachment is the best, and the retention of the retention force is the largest. ③ magnetic attachment initial and final retention force are the worst among the three groups. ④ titanium rod / titanium card attachment end retention of the best three groups, to maintain a better repair effect.
