患儿,男,2岁10个月.腹部进行性胀大一年半伴呼吸急促二月余,于1985年7月30日入院.缘于1984年元月发现患儿腹部明显肿大,无何不适,故其家长未予注意.随后见腹胀进行性加重,且于两月前出现呼吸急促,但无紫绀、腹痛、呕吐和黄疸.食欲如常,大便稍感不畅,但不干燥.曾拟诊为“腹水”,中医治疗无效.体检:T37.2℃、R40次/分、P132次/分.发育、营养一般,神清,呼吸急促,两肺可闻及少量中、小水泡音,心(一).全腹高度膨隆,呈蚌腹,上腹壁静脉轻度怒张,未见肠型.腹部张力较高,囊性感明显,无压痛、肝、脾触诊不满意.腹部叩诊均呈浊音,胁腹部鼓音并可闻及稀少肠呜音,有振水感,但移动性浊音(一).腹围86Cm,前后径25Cm,左右径24Cm,四肢无浮肿.
