目的了解贵州省贵阳市部分事业单位在职及退休女职工乳腺癌发病情况,初步探讨适合贵阳市事业单位妇女乳腺癌筛查的适宜技术。方法选取2011年6月至2102年5月间贵阳市12089例事业单位在职和退休女职工进行乳腺癌筛查,筛查方法以专科体检(CBE)和高频彩超(HFCDU)为主,数字钼靶X线(FFDM)作为筛查方法的补充,以病理活检为金标准,计算并比较CBE、HFCDU和CBE联合HFCDU的灵敏度、特异度、正确诊断指数和诊断指数。结果筛查共发现乳腺癌10例,总检出率82/10万,10例都为早期乳腺癌(包括0期和Ⅰ期),发病年龄为34~70岁,高发年龄为40~60岁,中位年龄46.5岁。比较各年龄段乳腺癌检出率,差异有统计学意义(P<0.05)。统计学分析比较3种筛查方式的灵敏度,差异无统计学意义(P>0.05),3种筛查方法特异度差异有统计学意义(P<0.05),其中专科体检联合高频彩超的特异度、正确诊断指数和诊断指数最高。结论2011至2102年间贵州省贵阳市部分事业单位在职及退休女职工乳腺癌检出率82/10万,高发年龄为40~60岁。CBE联合HFCDU作为贵阳市事业单位妇女乳腺癌筛查手段是可行的,联合FFDM可提高诊断率。
Objective To understand the incidence of breast cancer among working and retired female employees in some institutions in Guiyang City, Guizhou Province, and to explore suitable techniques suitable for screening breast cancer in institutions of Guiyang. Methods Breast cancer screening was conducted among active and retired female employees in 12,089 institutions in Guiyang from June 2011 to May 2102. The screening methods were mainly specialist examination (CBE) and high frequency color echocardiography (HFCDU), and digital molybdenum Target X-ray (FFDM) as a complement to screening methods, pathological biopsy as the gold standard, calculated and compared CBE, HFCDU and CBE with HFCDU sensitivity, specificity, correct diagnosis index and diagnostic index. Results A total of 10 cases of breast cancer were found in the screening, with a total detection rate of 82/10 million. All 10 cases were early stage breast cancer (stage 0 and stage Ⅰ). The age of onset was 34-70 years and the age of onset was 40-60 years , The median age of 46.5 years old. Comparing the detection rate of breast cancer in all age groups, the difference was statistically significant (P <0.05). Statistical analysis compared the sensitivity of the three screening methods, the difference was not statistically significant (P> 0.05), the specificity of the three screening methods was statistically significant (P <0.05), of which the special physical examination combined with high-frequency ultrasound Degree, the correct diagnosis index and the highest diagnostic index. Conclusion From 2011 to 2102, the detection rate of breast cancer in working and retired women employees in some institutions in Guiyang City, Guizhou Province was 82/10 million with a high incidence of 40-60 years. CBE combined HFCDU as a means of breast cancer screening institutions in Guiyang is feasible, combined with FFDM can improve the diagnostic rate.
