[目的]探讨延边地区小学生胰岛素抵抗(IR)对不同代谢危险因素及其聚集性的影响.[方法]采用整群抽样方法抽取延边地区朝鲜族和汉族小学生2 945例作为研究对象,调查一般情况,测定腰围、血压、空腹血糖、三酰甘油(TG)、高密度脂蛋白胆固醇及胰岛素水平.[结果]延边地区IR小学生腹型肥胖、高TG血症、高血压、高血糖及代谢综合征(MS)患病率分别为27.6%,40.0%,33.4%,8.3%及12.2%,与正常对照组比较差异均有统计学意义(P<0.001).朝鲜族小学生中0,1,2种以上代谢危险因素异常者IR检出率分别为6.0%,13.4%及25.1%,汉族小学生分别为4.8%,7.7%及3.3%;朝鲜族小学生IR与代谢危险因素聚集强度密切相关(P<0.001).[结论]延边地区小学生IR与不同代谢危险因素及MS密切相关,IR对不同代谢危险因素及MS患病危险的影响程度有所不同.
[Objective] To explore the effects of insulin resistance (IR) on different metabolic risk factors and their clustering in primary school students in Yanbian area. [Methods] A total of 2945 Korean and Han primary school children in Yanbian area were collected by cluster sampling method. The general situation , Waist circumference, blood pressure, fasting blood glucose, triglyceride (TG), high density lipoprotein cholesterol and insulin levels were measured. [Results] The abdominal fat, hypercholesteremia, hypertension, hyperglycemia and metabolic syndrome (MS) were 27.6%, 40.0%, 33.4%, 8.3% and 12.2%, respectively, which were significantly different from those of the normal control group (P <0.001) The detection rate of IR was 6.0%, 13.4% and 25.1% respectively for the above abnormal metabolic risk factors, and 4.8%, 7.7% and 3.3% for Han primary school children respectively. The IR intensity of Korean pupils was closely related to the aggregation intensity of metabolic risk factors (P <0.001) ). [Conclusion] The IR of primary school students in Yanbian is closely related to different metabolic risk factors and MS, and the influence of IR on different metabolic risk factors and the risk of MS is different.
