[目的]回顾性分析脐带绕颈产妇不同分娩方式对分娩结局的影响。[方法]回顾性病历对照研究。选取近1年内某院分娩全部脐带绕颈产妇,按不同分娩方式进行分组对照。并抽取同期经阴分娩的无脐绕颈的产妇,与有脐绕颈经阴分娩的产妇进行对照分析。比较其分娩结局。[结果]有脐带绕颈的产妇经阴分娩和剖宫产比较,新生儿评分和窒息的人数比较均无统计学差异,两组中均未出现新生儿死亡病例。经阴分娩的有脐绕颈者与无脐绕颈者比较,两组的新生儿窒息的发生率无统计学差异。有脐绕颈者产程时间短于有脐绕颈者,有统计学差异。[结论]在严格的产科监护下,有脐绕颈者经阴分娩与剖宫产比较,并不增加新生儿窒息的发生率。有脐带绕颈与无脐带绕颈者经阴分娩比较,新生儿窒息数亦无统计学差异。应理性的对待脐带绕颈这一个自然现象,排除高危因素,给予大多数的产妇以自然试产的机会,避免盲目的扩大应用剖宫产手术。
[Objective] To retrospectively analyze the effect of different modes of delivery on the delivery outcomes of umbilical cord around the neck. [Methods] A retrospective case history study. Select nearly a year hospital delivery of all umbilical cord around the neck maternal, according to different modes of delivery for group control. And draw the same period of vaginal delivery around the neck without the umbilical maternal, umbilical and neck around the vaginal delivery of mothers for controlled analysis. Compare their delivery outcomes. [Results] Compared with cesarean section, there was no significant difference in neonatal score and asphyxia between maternal umbilical cord wound with neck and cesarean section. Neonatal deaths were not found in both groups. There was no significant difference in the incidence of neonatal asphyxia between the two groups of umbilical patients who underwent vaginal delivery and umbilical patients. There are umbilical patients with neck length of labor is shorter than the umbilical neck, with statistical differences. [Conclusion] Under strict obstetric supervision, the umbilical patients with vaginal delivery and cesarean delivery did not increase the incidence of neonatal asphyxia. There are umbilical cord around the neck with no vaginal delivery around vaginal delivery comparison, the number of neonatal asphyxia no significant difference. Reasonable treatment of the umbilical cord around the neck of this natural phenomenon, excluding high-risk factors, giving the majority of mothers the opportunity of natural trial production, to avoid the blind expansion of the application of cesarean section.
