背景:随着现代医学的发展,单纯用发病率、存活率、死亡率等指标已不能全面且满意的评价脑膜瘤手术治疗效果。生活质量是一个与健康有关的多变量指标,能为医生提供来自患者生理、心理、社会适应等方面综合信息。脑膜瘤生活质量的测定可能是指导手术治疗、减少肿瘤复发较完善的评价方法之一。目的:探讨脑膜瘤患者的手术相关生活质量,为全面评估脑膜瘤手术提供相应的依据。设计:单一样本调查。单位:南京医科大学附属无锡市第二人民医院神经外科。对象:纳入南京医科大学附属无锡二院神经外科1995-01/2001-01收治脑膜瘤手术患者147例,其中男61例,女86例;男女比2∶3;年龄5~77岁,平均43岁,中位数43岁。方法:参照世界卫生组织生活质量100问卷及Karnofsky量表,设计脑膜瘤患者生活质量量表。对147例脑膜瘤患者横断面调查,并与正常人群进行比较,应用手术相关生活质量工作曲线分析确定生活质量满意值,并采用自身前后对照设计,分析手术对生活质量的影响。主要观察指标:脑膜瘤患者临床病史,放射学分析,手术级别,组织学特征及复发与生活质量的关系。结果:147例患者均进入结果分析。①脑膜瘤患者生活质量量表全项目的Cronbach’s系数为0.9521,相关系数为0.8685,提示脑膜瘤患者生活质量量表具有较好的信度和效度。②生活质量≥70分为获得满意生活质量。③除心理功能外脑膜瘤患者术后生理、独立性和日常生活能力明显改善(P<0.001)。结论:手术能使患者获得满意的生活质量;脑膜瘤患者生活质量测试有利于对脑膜瘤手术提供相应的依据。
Background: With the development of modern medicine, meningiomas have not been evaluated comprehensively and satisfactorily in surgical treatment of meningiomas simply with the indexes of incidence, survival and mortality. Quality of life is a health-related multivariate indicator that provides physicians with comprehensive information about the patient’s physiology, psychology, and social adjustment. The measurement of quality of life of meningiomas may be one of the better evaluation methods to guide the surgical treatment and reduce the recurrence of tumors. Objective: To explore the surgical related quality of life of patients with meningioma and to provide corresponding evidence for the comprehensive evaluation of meningioma surgery. Design: Single Sample Survey. SETTING: Department of Neurosurgery, Second People ’s Hospital of Wuxi City, Nanjing Medical University. PARTICIPANTS: A total of 147 patients with meningiomas undergoing surgery from January 1995 to January 2001 were enrolled in the Department of Neurosurgery, Wuxi Second Affiliated Hospital of Nanjing Medical University, including 61 males and 86 females; the ratio of men to women was 2: 3; and the average age was from 5 to 77 years The median age is 43. Methods: According to the World Health Organization Quality of Life Questionnaire 100 and Karnofsky Scale, the Quality of Life Scale for Meningioma Patients was designed. A total of 147 patients with meningioma were surveyed and compared with the normal population. Satisfaction values of quality of life were determined by surgery-related quality of life curve analysis. The quality of life was analyzed using self-control design. MAIN OUTCOME MEASURES: Clinical and medical history of meningiomas, radiological analysis, surgical grade, histological features and relapse and quality of life. Results: All 147 patients entered the result analysis. ① The quality of life scale of patients with meningioma whole project Cronbach’s coefficient of 0.9521, the correlation coefficient of 0.8685, suggesting that patients with meningioma quality of life scale has good reliability and validity. ② quality of life ≥ 70 divided into satisfactory quality of life. ③ In addition to psychological function of meningioma patients postoperative physiology, independence and daily living ability improved significantly (P <0.001). Conclusion: Surgery can make the patients get satisfactory quality of life. The quality of life test of meningioma patients is helpful to provide corresponding basis for meningioma surgery.
