目的:了解秦安县原发性高血压患者血压变异情况及血压变异对靶器官的损害,为血压晨峰患者早期预防靶器管损害提供一定的理论依据。方法:从该县不同年龄阶段的高血压患者中筛选出新发现的原发性高血压患者218例;既往明确诊断原发性高血压者258例。用动态血压器监测24小时血压情况,同时采用彩色多普勒超声检查仪测定患者的左室壁厚度、左室重量以及颈动脉厚度和斑块形成等指标;并测定患者的肝肾功能、血糖、血脂等指标;所有患者测身高、体质量、腰围,计算体质量指数。同时做24小时尿微量蛋白测定。结果:秦安县原发性高血压患者血压变异情况及血压变异对靶器管损害明显,尤其以晨峰时损害更甚。结论:原发性高血压晨峰者有更明显的颈动脉粥样硬化、左心室肥厚,同时有心肌复极时间的延长。
OBJECTIVE: To understand the variation of blood pressure and the variation of blood pressure in target organ in patients with essential hypertension in Qin’an County, and to provide a theoretical basis for the early prevention of damage of target tube in patients with blood pressure morning peak. METHODS: A total of 218 patients with newly diagnosed essential hypertension were screened from hypertensive patients of different ages in this county. Among them, 258 cases were diagnosed as essential hypertension in the past. The ambulatory blood pressure was used to monitor the blood pressure at 24 hours. The left ventricular wall thickness, left ventricular mass, carotid artery thickness and plaque formation were measured by color Doppler sonography. The liver and kidney function, , Blood lipids and other indicators; all patients height, weight, waist circumference, body mass index calculation. At the same time do 24-hour urine microalbumin determination. Results: The variability of blood pressure and blood pressure variation in patients with essential hypertension in Qin’an County had obvious damage to the target tube, especially when the morning peak was more severe. Conclusion: Morning peak of essential hypertension patients have more obvious carotid atherosclerosis, left ventricular hypertrophy, and prolonged myocardial repolarization time.
