目的探讨磁共振弥散加权成像和磁共振波普成像在脑胶质瘤鉴别诊断的临床价值。方法比较2010年1月至2013年2月间在安徽医科大学附属省立医院神经外科治疗的68例患者不同病理组在磁共振弥散加权成像和磁共振波普成像参数上的差异。结果高级别与低级别胶质瘤比较在Cho/NAA上差别有统计学意义(P=0.020);高级别胶质瘤与转移癌在Cho/NAA上有统计学差异(P=0.025);高级别胶质瘤与淋巴瘤在Cho/Cr上的差别有统计学意义(P=0.009);胶质瘤囊变区与脑脓肿在ADC值上有显著差异(P=0.006)。结论磁共振弥散加权成像联合磁共振波普成像可以辅助鉴别治疗前常规检查诊断不明的颅内占位疾病。
Objective To investigate the clinical value of diffusion-weighted MR imaging and magnetic resonance imaging in the differential diagnosis of glioma. Methods Sixty-eight patients with different pathology group who underwent neurosurgery at Provincial Hospital Affiliated to Anhui Medical University from January 2010 to February 2013 were compared with MRI diffusion weighted imaging and magnetic resonance imaging. Results There was a significant difference in Cho / NAA between high-grade and low-grade gliomas (P = 0.020). There was a significant difference in Cho / NAA between high-grade gliomas and metastatic carcinoma (P = 0.025) There were significant differences in Cho / Cr between grade glioma and lymphoma (P = 0.009). There was significant difference in ADC value between glioma cystic area and brain abscess (P = 0.006). Conclusion MR diffusion weighted imaging combined with magnetic resonance imaging can help identify preoperative diagnosis of unknown intracranial space-occupying diseases.
