目的通过对1例同时产生了类同种自身抗体和同种抗体患者的抗体分析及鉴定,了解Rh血型系统中同种抗体、简单特异性自身抗体和类同种自身抗体的血清学特征,从血清学上对三者进行区分,以便采取更适合的红细胞成分输血策略。方法采用筛选细胞和谱细胞、以不同方法对患者血清进行抗体筛查及鉴定。采用患者Rh抗体类似特异性对应抗原阴性的O型RhccDEE的献血者细胞、添加低离子液(LISS)的吸收方法,对患者吸收后的血清、红细胞放散液进行抗体鉴定。对以往相关文献进行回顾。结果患者Rh分型ccDEE,直抗阳性。血清抗体鉴定为IgM类的抗-C、抗-e和IgG类的抗-e(不排除抗-Ce可能性)。吸收后血清抗体鉴定为IgM类的抗-C、抗-e(不排除抗-Ce可能性)和IgG类的抗-e。放散液抗体鉴定为IgG类的抗-e。结论对患者红细胞Rh表型、直接抗球蛋白试验,以及分别对患者血清、吸收后血清及放散液抗体特异性进行综合分析,可区分出同种抗体或类同种抗体。而采用添加低离子液(LISS)的方法可更好的吸收低亲和力抗体。
OBJECTIVE: To understand the serological characteristics of alloantibodies, simple specific autoantibodies and their alloantibodies in Rh blood group system by analyzing and identifying one antibody in patients with same type of autoantibodies and alloantibodies. Serological distinction between the three in order to adopt a more appropriate red blood cell transfusion strategy. Methods The screening of cells and the cells of the spectrum were carried out. The serum of the patients was screened and identified by different methods. Antibody identification was performed on the serum and erythrocyte discharges absorbed by the patients using the absorption method of low ionic liquid (LISS) by donors of Rh type antigen of RhccDEE, which is similar to the antigen of Rh type in patients with Rh antigen. Review previous related literature. Results Patients with Rh typing ccDEE, direct anti-positive. Serum antibodies were identified as anti-c, anti-e and anti-e class of IgM (not excluding anti-Ce possibility). Serum antibodies after absorption were identified as anti-C, anti-e (not excluding anti-Ce possibility) IgM and anti-e IgG. Antibody to elution solution was identified as anti-e IgG class. Conclusion The analysis of the erythrocyte Rh phenotype, direct antiglobulin test, and the antibody specificity of the serum, absorption serum and effusion of the patients, respectively, can distinguish the same antibody or the same kind of antibody. The method of adding low ionic liquid (LISS) can better absorb low affinity antibody.
