应用全反式维甲酸(ATRA)治疗45例急性早幼粒细胞白血病(APL),分析治疗中白细胞数量对ATRA治疗APL的影响。结果显示:33例出现高白细胞症,发生率为73.3%。其中WBC<20×109/L者完全缓解率100%。加用化疗可延长获CR时间;>20×109/L者完全缓解率随WBC高峰值的升高而下降。提示粒细胞清除后给予小剂量ATRA、细胞毒药物及地塞米松联合治疗是治疗高白细胞症的一种有效措施。
All-trans retinoic acid (ATRA) was used to treat 45 patients with acute promyelocytic leukemia (APL), and the effect of ATRA on APL was analyzed. The results showed that 33 cases of high white blood cell disease occurred, the incidence was 73.3%. Among them, the complete remission rate of WBC<20×109/L was 100%. Adding chemotherapy can prolong the CR time; >20×109/L, the complete remission rate decreases with the increase of WBC peak value. The combination of low-dose ATRA, cytotoxic drugs, and dexamethasone administration after granulocyte clearance is an effective measure for the treatment of hyperleukocytosis.
