目的分析老年冠心病患者冠脉病变与超敏C反应蛋白(hs-CRP)及血浆脑钠肽(BNP)的相关性。方法 122例经冠状动脉造影确诊的老年冠心病,按临床类型分为急性心肌梗死(AMI)组32例,不稳定型心绞痛(UAP)组60例,稳定型心绞痛(SAP)组30例。按照不同病变血管数分为单支病变组、双支病变组、3支病变组。按照病变血管狭窄程度分为中度狭窄组、重度狭窄组、完全闭塞组。选择26例冠脉造影检查正常者为对照组。检测入院时血清hs-CRP及BNP浓度。结果 AMI、UAP、SAP组血浆hs-CRP及BNP浓度较对照组明显升高(P<0.01),AMI组hs-CRP及BNP浓度明显高于UAP组(P<0.01),UAP组明显高于SAP组(P<0.01)。三支病变组hs-CRP及BNP浓度显著高于双支、单支病变组(P<0.01),双支病变组高于单支病变组(P<0.01)。完全闭塞组hs-CRP及BNP浓度显著高于中度、高度狭窄组(P<0.01),高度狭窄组高于中度狭窄组(P<0.01)。结论 hs-CRP、BNP对老年冠心病的诊断及病变程度有预测价值。
Objective To analyze the correlation between coronary lesions and high-sensitivity C-reactive protein (hs-CRP) and plasma brain natriuretic peptide (BNP) in elderly patients with coronary heart disease. Methods A total of 122 elderly patients with coronary heart disease diagnosed by coronary angiography were divided into two groups: AMI group (60 cases), unstable angina pectoris group (UAP group) and stable angina pectoris group (30 cases). According to the number of different vascular lesions divided into single branch group, double branch disease group, 3 lesions group. According to the degree of vascular stenosis was divided into moderate stenosis group, severe stenosis group, complete occlusion group. Select 26 cases of normal coronary angiography as control group. Serum hs-CRP and BNP concentrations were measured at admission. Results The levels of plasma hs-CRP and BNP in AMI, UAP and SAP groups were significantly higher than those in control group (P <0.01). The concentrations of hs-CRP and BNP in AMI group were significantly higher than those in UAP group (P <0.01) SAP group (P <0.01). The levels of hs-CRP and BNP in the three lesions group were significantly higher than those in the double-branch and single-vessel lesion group (P <0.01). The double-vessel lesion group was higher than the single vessel lesion group (P <0.01). The levels of hs-CRP and BNP in patients with complete occlusion were significantly higher than those in patients with moderate and severe stenosis (P <0.01), those in patients with severe stenosis were significantly higher than those with moderate stenosis (P <0.01). Conclusion The predictive value of hs-CRP and BNP in the diagnosis and severity of coronary heart disease in elderly patients.
