[摘要] 目的 探究撳针耳穴埋针干预对全麻气管插管围术期病人血流动力学参数和自主神经功能影响。
方法 选择我院全麻气管插管手术病人200例以随机数字表法分为对照组与观察组,每组100例。对照组常规全麻处理,观察组在对照组基础上给予揿针耳穴埋针干预。比较两组病人麻醉诱导前(T0)、麻醉诱导期(T1)、插管即刻(T2)、术后(T3)、拔管时(T4)血流动力学参数(心率(HR)、平均动脉压(MAP)、外周血管阻力(SVR)、血流加速度指数(ACI)和心排血量(CO)),麻醉后自主呼吸恢复时间及拔管时间,术后6、12、24、48 h不同时间点疼痛程度(视觉模拟疼痛评分法,VAS)和不良反应等指标。
结果 对照组病人各时间点HR、MAP、SVR、ACI和CO比较,差异有显著性(F=8.756~161.493,P均<0.05),不同时间点两两比较结果显示对照组T2、T4的HR、MAP、SVR、ACI和CO均明显高于T0(P均<0.05),而T1各指标均显著低于T0(P均<0.05);观察组T2与T4的MAP、SVR和ACI均低于对照组(P均<0.05)。观察组麻醉后自主呼吸恢复时间及拔管时间均短于对照组(t=13.511、15.380,P均<0.01),术后6、12、24、48 h的VAS评分均低于对照组(F=6.937~252.890,P均<0.05)。两组病人恶心呕吐不良反应率相当(χ2=0.096,P>0.05),观察组病人术后躁动及呛咳发生率低于对照组(χ2=5.838、6.105,P均<0.05)。
结论 揿针耳穴埋针对全麻气管插管围术期病人具有积极影响,病人血流动力学参数改善,术后疼痛轻,恢复快。

EFFECT OF EAR ACUPUNCTURE ON HEMODYNAMIC PARAMETERS AND AUTONOMIC NERVE FUNCTION OF PATIENTS WITH TRACHEAL INTUBATION DURING PERIOPERATIVE PERIOD UNDER GENERAL ANESTHESIA

[ABSTRACT] Objective To explore the effect of ear acupuncture acupuncture on perioperative patients with tracheal intubation under general anesthesia.
Methods Two hundred patients with tracheal intubation under general anesthesia in our hospital were randomly divided into control group and observation group by random number table method,100 patients in each group. The control group was treated with general anesthesia, while the observation group was treated with ear acupuncture on the basis of the control group. Comparison between the two groups of patients before anesthesia induction (T0), anesthesia induction period (T1), immediate intubation (T2), postoperative (T3), when the tube drawing (T4) hemodynamic parameters (heart rate (HR), mean arterial pressure (MAP), peripheral vascular resistance (SVR), blood flow acceleration index (ACI) and cardiac output (CO)), patients after anesthesia spontaneous breathing recovery time and extubation time, After 6, 12, 24, 48 h different time points of pain (visual analogue pain scale, VAS), patients adverse reactions and other indicators.
