目的探讨果糖胺/白蛋白(FMN/ALB)和果糖胺/总蛋白(FMN/TP)比值对糖尿病的诊断价值。方法设糖尿病组(DM)413例、健康对照组(NGT)102名,检测两组外周血FMN、FPG、2hPG、ALB、TP、HbA1c、TG、TC水平,计算FMN/ALB、FMN/TP,比较组间各指标差异。应用受试者工作特征曲线(ROC)评价各指标对糖尿病的诊断效能。结果诊断糖尿病曲线下面积(AUC)AUC2hPG最大为0.988,AUCFMN/ALB0.968、AUCFMN/TP0.965、AUCHbA1c0.956、AUCFPG0.933、AUCFMN0.860;2 hPG的敏感性最高为96.6%,FMN/ALB 89.8%、FMN/TP 88.4%、HbA1c 86.0%、FPG84.7%、FMN 71.7%;HbA1c的特异性最高为100%,FPG 98.0%、2hPG 97.1%、FMN/TP 96.1%、FMN/ALB 95.1%、FMN 92.2%。结论 FMN/ALB和FMN/TP能排除血清蛋白浓度对FMN水平的影响,提高诊断效能。
Objective To investigate the diagnostic value of fructosamine / albumin (FMN / ALB) and fructosamine / total protein (FMN / TP) ratio in the diagnosis of diabetes mellitus. Methods FMN, ALB, TP, HbA1c, TG and TC were measured in peripheral blood of 413 patients with diabetes mellitus (DM) and 102 healthy controls (NGT) Compare the differences between the indicators of the group. The receiver operating characteristic curve (ROC) was used to evaluate the diagnostic efficacy of each index on diabetes. Results The area under curve (AUC) of AUC2hPG for diagnosis of diabetes was 0.988, AUCFMN / ALB0.968, AUCFMN / TP0.965, AUCHbA1c0.956, AUCFPG0.933 and AUCFMN0.860 respectively. The sensitivity of 2 hPG was 96.6% ALB 89.8%, FMN / TP 88.4%, HbA1c 86.0%, FPG 84.7%, FMN 71.7%; HbA1c with a specificity of up to 100%, FPG 98.0%, 2hPG 97.1%, FMN / TP 96.1%, FMN / %, FMN 92.2%. Conclusion FMN / ALB and FMN / TP can exclude the influence of serum protein concentration on FMN and improve the diagnostic efficiency.
