目的分析脂蛋白相关磷脂酶A_2(Lp-PLA_2)检测在老年心脑血管病防治中的意义。方法对该院神经内科80例住院患者进行观察,采集患者的血液标本,利用双抗体夹心免疫层析法联合光免疫分析仪配套对血浆中的Lp-PLA2水平进行检测。同时询问和记录患者的用药史、疾病史、家族史、血糖、糖化血红蛋白、血脂等相关信息,半年后电话随访,观察受检者出现心脑血管疾病情况。结果 80例老年受检者中,Lp-PLA_2>223ng/ml有6例,Lp-PLA_2在200~223ng/ml有41例,Lp-PLA_2<200ng/ml有33例,半年后电话随访结果显示,Lp-PLA_2预测存在重度心脑血管疾病风险的6例患者出现心脑血管疾病,分别为心肌梗死1例,心力衰竭2例,心绞痛1例,高血压脑病1例,脑卒中1例。结论对老年体检人群采用Lp-PLA_2检测可发现潜在心脑血管疾病的危险因素,可作为老年心脑血管疾病的风险评估指标,有利于及时采取治疗措施。
Objective To analyze the significance of detecting lipoprotein-associated phospholipase A 2 (Lp-PLA 2) in the prevention and treatment of cardiovascular and cerebrovascular diseases in the elderly. Methods 80 hospitalized patients in neurology department of the hospital were observed. The blood samples of patients were collected. The levels of Lp-PLA2 in plasma were detected by double antibody sandwich immunoassay and optical immunoassay analyzer. At the same time, the medical history, history of illness, family history, blood glucose, glycosylated hemoglobin, blood lipids and other related information of patients were asked and recorded. After half a year of telephone follow-up, the subjects were observed cardiovascular and cerebrovascular diseases. Results Sixty cases of Lp-PLA_2> 223ng / ml, Lp-PLA_2 of 200 ~ 223ng / ml in 41 cases and 33 cases of Lp-PLA_2 <200ng / ml in 80 elderly subjects were followed up Lp-PLA_2 predicted cardio-cerebrovascular disease in 6 patients at risk of severe cardiovascular and cerebrovascular diseases, including 1 case of myocardial infarction, 2 cases of heart failure, 1 case of angina pectoris, 1 case of hypertensive encephalopathy and 1 case of stroke. Conclusion Lp-PLA 2 test can detect the risk factors of potential cardiocerebral vascular diseases in elderly medical examination population, which can be used as risk assessment index of elderly cardiovascular and cerebrovascular diseases, which is conducive to timely treatment.
