作者报告1例因输注低分子右旋糖酐致急性肾衰,并使血浆胶体渗透压升高.是罕见的并发症.病者男,46岁。因脑卒中后1周少尿性急性肾衰由神经科转入监护病房.血肌酐15.4mg/dl,血尿素氮132mg/dl,血浆胶体渗透压34(正常值22.2±2.5)mmHg,血浆蛋白正常,这是由于每日输注10%低分子旋糖酐500ml(总量350g)所致.作去除胶质治疗后,血浆胶体渗透压降至25.1mmHg。入院后第五天于多尿期前施血液透析3次.10天后血肌酐降
The authors report a rare complication of acute renal failure caused by infusion of low-molecular dextran and increased plasma colloid osmotic pressure. One week after the stroke, oliguric acute renal failure was transferred from neurology department to the intensive care unit.Serum creatinine 15.4mg / dl, blood urea nitrogen 132mg / dl, plasma colloid osmotic pressure 34 (normal 22.2 ± 2.5) mmHg, plasma protein Normal, due to a daily infusion of 500 ml of 10% low molecular weight dextran (350g total). After removal of the glial therapy, the plasma colloid osmotic pressure dropped to 25.1mmHg.
