目的探讨联合检测血清中抗环瓜氨酸肽抗体(抗-CCP)、抗角蛋白抗体(AKA)与类风湿因子(RF)对类风湿性关节炎(RA)的诊断意义。方法分别采用酶联免疫吸附法(ELISA)法、间接免疫荧光法和速率散射比浊法测定96例RA患者和40例健康体检者的抗-CCP、AKA、RF水平,比较3种抗体单独和联合检测对诊断RA的敏感度和特异度。结果 RA组中的抗-CCP、AKA和RF的阳性率分别为58.3%、40.6%和67.7%。对RA诊断的敏感度、特异度分别为:抗-CCP:67.7%、89.2%;AKA:47.5%、86.5%;RF:74.6%、73.2%。3项指标联合检测的敏感度、特异度分别为90.3%、91.6%。抗-CCP与AKA、RF之间显著相关(P<0.05)。结论抗-CCP、AKA及RF的联合检测可使类风湿性关节炎的诊断敏感度和特异度得到有效提高。
Objective To investigate the diagnostic value of combined detection of anti-cyclic citrullinated peptide antibody (anti-CCP), anti-keratin antibody (AKA) and rheumatoid factor (RF) in rheumatoid arthritis (RA) Methods The levels of anti-CCP, AKA and RF in 96 patients with RA and 40 healthy subjects were measured by enzyme linked immunosorbent assay (ELISA), indirect immunofluorescence and rate nephelometry, respectively. The levels of anti-CCP, Sensitivity and specificity of joint detection in the diagnosis of RA. Results The positive rates of anti-CCP, AKA and RF in RA group were 58.3%, 40.6% and 67.7% respectively. The sensitivity and specificity for the diagnosis of RA were: anti-CCP: 67.7%, 89.2%; AKA: 47.5%, 86.5%; RF: 74.6%, 73.2%. The sensitivities and specificities of the three tests were 90.3% and 91.6% respectively. There was a significant correlation between anti-CCP and AKA and RF (P <0.05). Conclusion The combined detection of anti-CCP, AKA and RF can effectively improve the diagnostic sensitivity and specificity of rheumatoid arthritis.
