目的探讨体外受精-胚胎移植(IVF-EF)妊娠的心脏病胎儿超声心动图特征。方法孕18~24周行胎儿超声心动图检查的IVF-EF妊娠孕妇1138例,分析IVF-EF妊娠胎儿先天性心脏病超声表现及分类特征,并跟踪所有孕妇的妊娠过程及胎儿临床结局。结果本组病例共1138例,其中单胎妊娠959例,双胎妊娠179例,共检测胎儿1317个,经检查发现超声心动图诊断的胎儿心脏畸形共16例,心脏畸形发生率为1.2%(16/1317),以上16例心脏畸形胎儿均经过临床确诊。具体如下:法洛四联症6例,完全型房室间隔缺损2例,主动脉狭窄及缩窄各1例,肺动脉狭窄2例,室间隔缺损4例。结论 IVF-EF妊娠胎儿先天性心脏病的发生率高,其中以法洛四联症和室间隔缺损最多见。
Objective To investigate the echocardiographic features of fetuses with heart disease undergoing in vitro fertilization-embryo transfer (IVF-EF) pregnancy. Methods A total of 1138 IVF-EF pregnant women undergoing fetal echocardiography from 18 to 24 weeks of gestation were enrolled in this study. The ultrasonographic features and classification of IVF-EF fetuses with congenital heart disease were analyzed. All pregnant women were followed up for pregnancy and fetus clinical outcomes. Results A total of 1138 cases of this group of patients, including 959 cases of singleton pregnancies, 179 cases of twin pregnancies, a total of 1317 fetuses were detected, after examination revealed echocardiography diagnosed a total of 16 cases of fetal cardiac malformations, the incidence of cardiac malformations was 1.2% ( 16/1317), more than 16 cases of cardiac malformation fetus have been clinically diagnosed. Specifically as follows: tetralogy of Fallot in 6 cases, 2 cases of complete atrioventricular septal defect, 1 case of aortic stenosis and narrowing, 2 cases of pulmonary stenosis, ventricular septal defect in 4 cases. Conclusion The incidence of congenital heart disease in IVF-EF fetuses is high, of which tetralogy of Fallot and ventricular septal defect are the most common.
