我们对100例足月妊娠头位第一胎分娩者,在产程活跃早期推阿托品1mg,同时配合人工破膜、应用催产素。结果与对照组相比,其宫口扩张速度明显加快(P<0.01),活跃期时间明显缩短(P<0.01)。 头位正常分娩,产程进展快慢,主要决定于产力、胎头下降速度及宫颈松紧度。活跃期应用阿托而能抑制子宫颈网状神经纤维及肾上腺素或去甲肾上腺素对宫颈的兴奋,使宫颈松驰扩张。此时配合人工破膜、应用催产素,可调节有效产力,只要胎头下降无阻力,活跃期产程进展很快。
