目的观察第一代冷冻球囊消融术(CB)在治疗阵发性心房颤动(阵发房颤)中的安全性及有效性情况。方法回顾性分析因阵发房颤于医院行第一代CB患者50例(CB组),导管射频消融术(RF)患者50例(RF组),比较2组治疗效果及复发率。结果 2组术中即刻消融成功率均为100%,CB组手术持续时间为(94±24)min明显短于RF组的(126±23)min,差异有统计学意义(t=8.625,P<0.05)。CB组的50位患者共计肺静脉193支,平均每位患者消融(10.0±2.4)次。术后除CB组发生1例膈神经麻痹外,未出现其他并发症。2组患者早期及晚期复发率比较差异均无统计学意义(P>0.05)。结论第一代CB的早期复发率及晚期复发率均达到了国际标准水平,安全性上未发现严重的心包积液、心包填塞、心房食道瘘、血栓栓塞及穿刺血管损伤等并发症,膈神经麻痹不影响远期预后。
Objective To observe the safety and efficacy of the first generation of frozen balloon ablation (CB) in the treatment of paroxysmal atrial fibrillation (paroxysmal atrial fibrillation). Methods Fifty patients (CB group) and 50 patients (RF group) underwent radiofrequency catheter ablation (RF) were retrospectively analyzed. The therapeutic effect and recurrence rate were compared between the two groups. Results The success rate of immediate ablation in both groups was 100%, and the duration of operation in CB group was (94 ± 24) min which was significantly shorter than that in RF group (126 ± 23) min (t = 8.625, P <0.05). Of the 50 patients in CB group, 193 had pulmonary veins, with an average of 10.0 ± 2.4 in each patient. One case of phrenic nerve paralysis occurred in CB group after operation, and no other complications occurred. There was no significant difference in the early and late recurrence rates between the two groups (P> 0.05). CONCLUSION: The first-generation CB with early recurrence rate and late recurrence rate reached the international standard level. No serious complications such as pericardial effusion, pericardial tamponade, atelectasis, thromboembolism and puncture vascular injury were found in the safety. The phrenic nerve Paralysis does not affect long-term prognosis.
