目的探讨胃癌全胃切除结肠代胃的临床效果。方法选取获嘉县人民医院2012年6月至2013年6月收治的98例进行胃癌全胃切除手术的患者为研究对象,将其随机分为A组与B组,每组49例。A组给予结肠代胃的消化道重建方式进行治疗,B组给予食管空肠吻合术的消化道重建方式进行治疗,对两组患者的临床效果进行比较。结果 A组术后并发症发生率明显较B组低,A组发生营养不良的例数明显较B组少,且A组术后胃肠食物储量明显较B组高,差异均有统计学意义(P<0.05)。术后随访两组患者生存率均较高,胃癌复发率均较低,差异无统计学意义(P>0.05)。结论胃癌全胃切除结肠代胃术能有效提高患者食物储备量,降低术后并发症发生率,提高患者整体的生存质量,在全胃切除术中是较为理想的消化道重建方式,值得临床应用推广。
Objective To investigate the clinical effect of total gastrectomy for gastric cancer on behalf of the stomach. Methods A total of 98 patients who undergone gastrectomy for gastric cancer were selected from the People’s Hospital of Huaxia County from June 2012 to June 2013. The patients were randomly divided into A group and B group, with 49 cases in each group. A group given to the stomach on behalf of the gastrointestinal tract reconstruction of the way for treatment, B group given esophageal jejunostomy gastrointestinal reconstruction for treatment, the clinical effect of the two groups were compared. Results The incidence of postoperative complications in group A was significantly lower than that in group B. The number of malnutrition in group A was significantly less than that in group B, and the postoperative gastrointestinal food reserves in group A were significantly higher than those in group B. The difference was statistically significant (P <0.05). Postoperative follow-up of both groups were higher survival rates, recurrence rate of gastric cancer were lower, the difference was not statistically significant (P> 0.05). Conclusion Gastric gastrectomy and colorectal gastrogavage can effectively increase the patient’s food reserves, reduce the incidence of postoperative complications and improve the overall quality of life of patients, which is an ideal method of gastrectomy in total gastrectomy and is worthy of clinical application Promotion.
