目的:探讨阿卡波糖联合益心舒胶囊治疗糖尿病心肌病的临床疗效及其对患者血清结缔组织生长因子(CTGF)、高敏 C 反应蛋白(hs-CRP)的影响。方法选择糖尿病心肌病患者56例,采用数字表法随机分为对照组28例、观察组28例,两组均给予对症处理,皮下注射胰岛素,予以阿司匹林、缬沙坦干预,观察组在此基础上加用阿卡波糖联合益心舒胶囊治疗,比较两组治疗前后总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBG)、餐后2 h 血糖(PBG)、糖化血红蛋白(HbA1c)、hs-CRP 及 CTGF 等指标的变化,采用超声心动图测定治疗前后两组心功能的改善情况,比较两组治疗效果。结果治疗后,两组 TC、LDL-C 均降低,与治疗前比较差异均有统计学意义(t =14.024、27.191,25.679、60.852,均 P <0.05),观察组 TC、LDL-C 降低幅度高于对照组[观察组:(4.71±0.33)mmol/L、(2.41±0.16)mmol/L,对照组:(6.01±0.06)mmol/L、(3.94±0.12)mmol/L];观察组治疗后 HDL-C [(1.77±0.11)mmol/L]上升,与对照组[(1.02±0.06)mmol/L]相比差异有统计学意义(t =31.673,P <0.05)。治疗后,观察组 HbA1c、FBG、PBG 分别为(6.62±0.86)%、(5.62±1.20)mmol/L、(8.00±2.21)mmol/L,对照组分别为(7.48±1.13)%、(6.81±1.26)mmol/L、(9.83±3.25)mmol/L,两组差异均有统计学意义(t =3.204、3.618、2.463,均 P <0.05)。治疗后,观察组 hs-CRP、CTGF 分别为(11.26±5.21)mg/mL、(216.52±72.12)ng/L,对照组分别为(18.21±4.57)mg/mL、(428.46±62.78)ng/L,两组差异均有统计学意义(t =11.728、5.306,均 P <0.05)。治疗后,观察组 LVEDD 降低幅度高于对照组[(5.33±0.21)cm 比(6.12±0.36)cm],LVEF、E /A 峰上升幅度高于对照组[(51.63±2.25)%、(1.83±0.04)比(45.26±3.16)%、(1.12±0.15)](t =10.030、8.689、24.200,均 P <0.05),两组治疗前后 IVSD 差异无统计学意义(P >0.05)。观察组治疗有效率(92.86%)明显高于对照组(67.86%)(χ2=5.543,P <0.05)。结论采用阿卡波糖联合益心舒胶囊治疗糖尿病心肌病,治疗有效率高,患者血糖、血脂、心功能指标改善显著。“,”Objective To explore clinical efficacy of acarbose combined with Yixinshu capsule in treating diabetic cardiomyopathy and its effects on serum connective tissue growth factor(CTGF),high sensitive C reactive protein(hs -CRP)of patients.Methods 56 patients with diabetic cardiomyopathy were selected and divided by random number table into the control group and the observation group(each 28 cases),the two groups received expectant treatment,subcutaneous injection of insulin and intervention with aspirin and valsartan,based on this,the observation group was treated with acarbose combined with Yixinshu capsule,the changes of total cholesterol(TC),high density lipoprotein(HDL -C),low density lipoprotein(LDL -C),fasting blood glucose(FBG),postprandial 2h blood glucose (PBG),glycosylated hemoglobin(HbA1c),hsCRP,CTGF and other indexes before and after treatment were compared between the two groups,the improvement of heart function before and after treatment was determined by ultrasonic echocardiogram,the curative effects were compared between the two groups.Results After treatment,TC and LDL -C in the two groups were decreased significantly than those before treatment(t =14.024,27.191,25.679,60.852,all P <0.05),the decreasing amplitudes of TC and LDL -C in the observation group were higher than those in the control group[the observation group:(4.71 ±0.33)mmol/L,(2.41 ±0.16)mmol/L,the control group:(6.01 ±0.06)mmol/L, (3.94 ±0.12)mmol/L];After treatment,HDL -C of the observation group[(1.77 ±0.11 )mmol/L]significantly increased compared with the control group [(1.02 ±0.06)mmol/L](t =31.673,P <0.05 ).After treatment, HbA1c,FBG and PBG of the observation group were(6.62 ±0.86)%,(5.62 ±1.20)mmol/L,(8.00 ±2.21)mmol/L respectively,the control group were(7.48 ±1.13)%,(6.81 ±1.26)mmol/L,(9.83 ±3.25)mmol/L respectively, there were significant differences between the two groups(t =3.204,3.618,2.463,all P <0.05).After treatment, hs -CRP and CTGF of the observation group were(11.26 ±5.21)mg/mL,(216.52 ±72.12)ng/L respectively,the control group were (18.21 ±4.57)mg/mL,(428.46 ±62.78)ng/L respectively,there were significant differences between the two groups(t =11.728,5.306,all P <0.05).The decreasing amplitude of LVEDD in the observation group after treatment was higher than that in the control group[(5.33 ±0.21 )cm vs(6.12 ±0.36)cm],and the increasing amplitudes of LVEF and E /A peak were higher than those in the control group[(51.63 ±2.25)%,(1.83 ± 0.04)vs(45.26 ±3.16)%,(1.12 ±0.15)](t =10.030,8.689,24.200,all P 0.05).The effective rate of treatment in observation group(92.86%)was significantly higher than that in the control group(67.86%)(χ2 =5.543,P <0.05). Conclusion To use acarbose combined with Yixinshu capsule in the treatment of diabetic cardiomyopathy,the effective rate of treatment is high,the blood glucose,blood lipids and heart function indexes can be improved remarkably.
