目的 :评价细胞遗传学动态改变在判断慢性粒细胞白血病 (CML)病程进展中的作用 ,以及干扰素对CML细胞遗传学缓解率的影响。方法 :对 2 0例CML患者进行动态细胞遗传学检测。结果 :2 0例患者初诊时均具有 9号和 2 2号染色体易位 ,形成Ph染色体 ,2 0例患者在监测过程中 ,有 5例患者出现克隆转化 ,此 5例患者均在短期内急变。在应用干扰素治疗的 12例患者中有 4例患者在治疗中曾转变为正常核型 ,达细胞遗传学完全缓解。结论 :应用常规细胞遗传学检查对于诊断临床上不典型的CML有重要作用。在CML病程中动态监测细胞遗传学改变 ,如果发生克隆转化 ,可以预测急变 ,以便提早治疗。早期应用干扰素治疗CML可以使Ph染色体转阴 ,监测其动态变化可以指导临床治疗。
OBJECTIVE: To evaluate the role of changes in cytogenetics in determining the course of CML and the effect of interferon on CML cytogenetic response. Methods: Twenty cases of CML patients were detected by dynamic cytogenetics. Results: Twenty chromosomes had chromosome 9 and 2 2 translocation at the time of first visit, and Ph chromosome was formed. In the monitoring process, 5 patients had clonal transformation in 20 patients, all of which were rapidly changed in the short term . Of the 12 patients treated with interferon, 4 had converted to normal karyotype during treatment, achieving complete cytogenetic remission. CONCLUSIONS: The use of routine cytogenetic tests has an important role in the diagnosis of clinically atypical CML. Dynamic monitoring of cytogenetic changes during the course of CML, if clonal transformation occurs, can predict acute changes for early treatment. Early application of interferon treatment of CML can make Ph chromosome negative, monitoring its dynamic changes can guide clinical treatment.
