目的探讨经颅多普勒(TCD)在儿童偏头痛发作时所测定的颅底血管血流动力学的改变。分析儿童偏头痛的TCD特征、临床特点及治疗转归,提高对此病的认识。方法采用经颅多普勒(TCD)测量30例正常儿童的脑血管血流速度。同时对68例儿童偏头痛发作时颅内各血管的血流速度进行回顾性分析,将采集的数据储存于电脑中,进行频谱处理分析,再根据收缩期和舒张期流速自动分析得出平均血流速度。结果以30例健康儿童为对照组,68例偏头痛患儿中有52例血流速增快,6例血流速减慢,5例双侧流速不对称,5例前、后循环血流速快慢不相同。结论偏头痛儿童存在着脑血管舒缩功能障碍,通过TCD对偏头痛儿童的脑血管血流速度的检测,有助于偏头痛的诊断、鉴别诊断、药物选择及疗效评价。
Objective To investigate the changes of hemodynamics of skull base vessels measured by transcranial Doppler (TCD) in childhood migraine attacks. To analyze the TCD features, clinical characteristics and treatment outcomes of migraine in children and to improve the understanding of the disease. Methods Transcranial Doppler (TCD) was used to measure cerebrovascular blood flow velocity in 30 normal children. At the same time, retrospective analysis was made on the blood flow velocity of each intracranial blood vessel in 68 children with migraine attack. The collected data were stored in a computer and analyzed by frequency spectrum. The mean blood volume was automatically calculated according to the systolic and diastolic flow rate Flow rate. Results Thirty healthy children were used as control group. Among 68 cases of migraineurs, 52 cases of blood flow increased rapidly, 6 cases of blood flow slowed down, 5 cases of bilateral flow asymmetry, 5 cases of anterior and posterior circulation blood flow Speed is not the same. Conclusion There are cerebral vasomotor dysfunction in children with migraine. Detection of cerebrovascular blood flow velocity in migraine children by TCD is helpful for the diagnosis, differential diagnosis, drug selection and evaluation of the curative effect of migraine.
