目的:分析助产护理干预对高龄产妇分娩方式及产程的影响,评价助产护理干预价值。方法:选取2014年1月至2016年5月广东省惠州市第三人民医院接待的高龄产妇178例,据病号顺序,随机对照分组,将产妇分为对照组、观察组各89例,对照组按照产程新标准定义潜伏期、活跃期、第二产程,并采用相应的方法处理,常规护理,观察组在对照组基础上,给予助产护理干预。主要包括以人员选择、心理支持、健康教育、角色扮演与家属支持、营养管理、环境管理、舒适护理、选择合适的体位,对比分娩方式、产程。结果:观察组阴道分娩成功率91.01%高于对照组75.28%,转剖宫产率、产前助产率、药物干预率、侧切率低于对照组,差异有统计学意义(P<0.05);观察组活跃期、第一产程、第二产程时间低于对照组,差异具有统计学意义(P<0.05)。结论:助产护理干预有助于预防难产,缩短产程。
OBJECTIVE: To analyze the influence of midwifery nursing intervention on the mode of delivery and labor process of the elderly maternity women and evaluate the value of midwifery nursing intervention. Methods: From January 2014 to May 2016, 178 cases of elder mothers received by the Third People’s Hospital of Huizhou City, Guangdong Province were selected. According to the order of the disease and randomized control group, the mothers were divided into the control group and the observation group, with 89 cases in each group. Group according to the new standard of labor to define the incubation period, active phase, the second stage of labor, and use the appropriate method of treatment, routine care, observation group on the basis of the control group, giving midwifery care intervention. Mainly including the choice of personnel, psychological support, health education, role play and family support, nutrition management, environmental management, comfort and care, choose the right position, contrast mode of delivery, labor process. Results: The success rate of vaginal delivery in the observation group was 91.01% higher than that of the control group (75.28%). The cesarean section rate, prenatal delivery rate, drug intervention rate and lateral incision rate were lower in the observation group than those in the control group (P <0.05) ; The observation group active stage, the first stage of labor, the second stage of labor time is lower than the control group, the difference was statistically significant (P <0.05). Conclusion: Midwifery nursing intervention can help prevent dystocia and shorten labor.
