目的研究一组分离自消化内科的多重耐药(multidrug-resistant,MDR)鲍曼不动杆菌的耐药基因检测分析结果。方法收集分离自2014-06/2016-06月消化内科住院患者的MDR鲍曼不动杆菌共20株,聚合酶链式反应(polymerase chain reaction,PCR)扩增包括6种β-内酰胺酶耐药基因,9种氨基糖苷类修饰酶基因,6种氟喹诺酮类耐药基因,5种16S rRNA甲基化酶基因和2种外排泵基因共28种耐药基因,并对相关基因PCR产物进行测序分析。结果在20株MDR鲍曼不动杆菌均检测到blaOXA-23群,blaOXA-51群及blaADC三种β-内酰胺酶耐药基因。氨基糖苷类修饰酶基因aac(3)-I,aac(6’)-Ib,ant(3″)-I,aph(3’)-I的携带率分别为95.00%(19/20),95.00%(19/20),90.00%(18/20)和100%(20/20);9株细菌中检测到armA基因,检测到gyrA(Ser83Leu)和parC(Ser80Leu)突变。在20株细菌中均检测到adeB基因,在3株细菌种检测到qacEΔ1基因。结论本组MDR鲍曼不动杆菌携带的耐药基因和耐药表型相一致,携带blaOXA-23群、blaOXA-51群基因及blaADC基因,aac(3)-I,aac(6’)-Ib,ant(3″)-I,aph(3’)-I等4种修饰酶基因,armA基因,外排泵基因adeB,gyrA(Ser83Leu)及parC(Ser80Leu)的点突变是导致这一组鲍曼不动杆菌MDR的主要原因。
Objective To study the drug resistance of a group of multidrug-resistant (MDR) Acinetobacter baumannii isolated from gastroenterology. Methods A total of 20 strains of MDR Acinetobacter baumannii isolated from patients in the department of gastroenterology from 2014-06 to 2016-06 were collected. The polymerase chain reaction (PCR) amplification included six kinds of β-lactamase resistance A total of 28 kinds of drug resistance genes were obtained, including 28 genes of drug resistance genes, 9 kinds of aminoglycoside modified enzymes, 6 kinds of fluoroquinolone resistance genes, 5 kinds of 16S rRNA methylase genes and 2 efflux pump genes. The related gene PCR products Perform sequencing analysis. Results Three strains of blaOXA-23, blaOXA-51 and blaADC were detected in 20 strains of MDR Acinetobacter baumannii. The carrying rates of aminoglycoside modifying enzyme aac (3) -I, aac (6 ’) - Ib, ant (3 “) - I and aph (3’) - I were 95.00% (19/20) (19/20), 90.00% (18/20) and 100% (20/20) respectively.The armA gene was detected in 9 strains of bacteria and gyrA (Ser83Leu) and parC (Ser80Leu) were detected in 20 strains of bacteria All detected adeB gene and qacEΔ1 gene was detected in 3 strains of bacteria.Conclusion The MDR-resistant Acinetobacter baumannii strains in this group are consistent with the resistant gene and carry blaOXA-23 group, blaOXA-51 group gene and The genes of four kinds of modified enzymes, armA gene, effBc gene adeB, gyrA gene, blaACC gene, aac (3) -I, aac (6 ’) - Ib, ant (3 ”) - I and aph (3’) - (Ser83Leu) and parC (Ser80Leu) point mutation is the main cause of this group of A. baumannii MDR.
