目的探讨急性冠脉综合征(ACS)患者血清基质金属蛋白酶-9(MMP-9)、超敏C反应蛋白(hs-CRP)水平与冠状动脉病变的相关性。方法 52例ACS患者分为急性心肌梗死组(AMI组)26例和不稳定型心绞痛组(UAP组)26例,另选稳定型心绞痛组(SAP组)26例和正常对照组26例,检测各组血清MMP-9、hs-CRP水平。结果 AMI组血清MMP-9、hs-CRP水平显著高于UAP组、SAP组和对照组,组间比较差异有统计学意义(P﹤0.05);UAP组血清MMP-9、hs-CRP水平显著高于SAP组和对照组,组间比较差异有统计学意义(P﹤0.05);SAP组血清MMP-9、hs-CRP水平显著高于对照组,组间比较差异有统计学意义(P﹤0.05)。三支病变组血清MMP-9、hs-CRP水平显著高于双支病变组和单支病变组,组间比较差异有统计学意义(P﹤0.05);双支病变组血清MMP-9、hs-CRP水平显著高于单支病变组,组间比较差异有统计学意义(P﹤0.05)。血清MMP-9、hs-CRP水平与冠脉病变支数均呈显著正相关(r分别为0.352、0.313,P﹤0.05);与Gensini积分亦呈显著正相关(r值分别为0.473、0.418,P﹤0.05)。结论 MMP-9和hs-CRP均可作为评价冠状动脉病变程度的血清标志物,对冠脉斑块稳定性也有一定的预测价值。
Objective To investigate the correlation between the levels of serum matrix metalloproteinase-9 (MMP-9), hs-CRP and coronary artery disease in patients with acute coronary syndrome (ACS). Methods Totally 52 ACS patients were divided into acute myocardial infarction (AMI) group (26 cases) and unstable angina pectoris group (UAP group), 26 cases of stable angina pectoris group (SAP group) and 26 cases of normal control group The serum MMP-9, hs-CRP levels in each group. Results The levels of serum MMP-9 and hs-CRP in AMI group were significantly higher than those in UAP group, SAP group and control group (P <0.05). The levels of MMP-9 and hs-CRP in UAP group were significantly higher than those in UAP group (P <0.05). The levels of serum MMP-9 and hs-CRP in SAP group were significantly higher than those in control group, with significant difference between the two groups (P < 0.05). The levels of serum MMP-9 and hs-CRP in the three lesions group were significantly higher than those in the double-vessel disease group and single-vessel disease group, with significant difference between the two groups (P <0.05). The serum levels of MMP-9 and hs -CRP levels were significantly higher than the single-vessel disease group, the difference between the groups was statistically significant (P <0.05). The levels of serum MMP-9 and hs-CRP were positively correlated with the number of coronary lesions (r = 0.352,0.313, P <0.05), and positively correlated with Gensini score (r = 0.473,0.418, P <0.05). Conclusion Both MMP-9 and hs-CRP can be used as serum markers to evaluate the severity of coronary artery disease, and have certain predictive value for the stability of coronary artery plaque.
