目的通过使用餐前胰岛素相同而基础胰岛素不同的强化方案———既地特胰岛素(长效)联合生物合成人胰岛素注射液R(诺和灵R)与精蛋白生物合成人胰岛素注射液N(诺和灵N中效)联合诺和灵R治疗2型糖尿病。对地特胰岛素与诺和灵N在强化方案中的疗效进行比较分析。方法将符合纳入标准的50例2型糖尿病患者随机分为地特和诺和两组,每组各25例。地特组给予地特胰岛素联合诺和灵R,诺和组给予诺和灵N联合诺和灵R治疗。观察两组治疗前后血糖控制情况、血糖达标所用时间、达标时胰岛素用量及低血糖发生情况。结果两组空腹血糖(FPG)、餐后血糖(2h-PPG)同治疗前相比较均有显著下降,差异有统计学意义(P<0.01),但治疗后组间血糖比较差异无统计学意义(P>0.05);与诺和组比较,地特组血糖达标时间短(P<0.05),达标时胰岛素用量少(P<0.05),低血糖率低(P<0.01)。结论地特胰岛素联合诺和灵R与诺和灵N联合诺和灵R强化治疗2型糖尿病,有效性相似,但地特组血糖达标快,低血糖风险低,且胰岛素用量少。
OBJECTIVES: To compare the effect of basal insulin, a combination of both insulin (long-acting) and human biosynthetic human insulin, R (norvepinal R), and protamine biosynthesis of human insulin, N Novolin N in effect) and Novolin R treatment of type 2 diabetes. Comparative study of the effect of detemir and Novolin N in intensive regimens. Methods Fifty patients with type 2 diabetes who met the inclusion criteria were randomly divided into two groups, Termitol and Conrol, each with 25 cases. Dexamethasone given dextrins combined with norphelin R, Novo group and noradrenal N uniconazole R treatment. Before and after treatment, the control of blood glucose, the time of reaching the blood glucose level, the dosage of insulin and the incidence of hypoglycemia were observed before and after treatment. Results The fasting blood glucose (FPG) and postprandial blood glucose (2h-PPG) in both groups were significantly lower than those before treatment (P <0.01), but there was no significant difference in blood glucose between the two groups after treatment (P> 0.05). Compared with Nuohe group, the time of reaching the standard of glycemic control in Dete group was shorter (P <0.05), the dosage of insulin was less (P <0.05) and the rate of hypoglycemia was lower (P <0.01). Conclusion Combination of detemir insulin and norprazole with Novolin and N combined with Novolin R has the similar efficacy in type 2 diabetes. However, the dextran group has fast blood glucose, low risk of hypoglycemia, and low insulin dosage.
