目的比较Smartcare智能脱机模式与自主呼吸试验在慢性阻塞性肺疾病(COPD)患者撤机中的效果,评估Smartcare智能脱机模式应用于COPD撤机过程中的作用。方法选择2009年1月至2013年11月收治的COPD机械通气满足撤机条件的患者40例纳入观察,随机分为Smartcare智能脱机模式组(n=23)与自主呼吸试验组(n=17)。记录每例患者的机械通气时间、撤机时间、脱机成功率、48 h再插管率及呼吸机相关性肺炎发生率。结果与自主呼吸试验相比,Smartcare智能脱机模式能明显降低COPD患者的机械通气时间及撤机时间(P均<0.05),二者撤机成功率、48 h再插管率与呼吸机相关性肺炎发生率无统计学差异(P均>0.05)。结论 Smartcare智能脱机模式可缩短COPD患者机械通气时间与撤机时间,是一种有效且安全的撤机方式。
Objectives To compare the effectiveness of Smartcare Smart Off-line and Spontaneous Breath Test in weaning patients with chronic obstructive pulmonary disease (COPD) and evaluate the role of Smartcare Smart Off-line in COPD weaning. Methods Forty patients with COPD ventilated patients who met the weaning condition from January 2009 to November 2013 were included in the study. They were randomly divided into two groups: the Smartcare intelligent offline model group (n = 23) and the spontaneous breathing test group (n = 17) ). The time of mechanical ventilation, the weaning time, the success rate of off-line, the re-intubation rate at 48 hours and the incidence of ventilator-associated pneumonia were recorded in each patient. Results Compared with spontaneous breathing test, Smartcare intelligent off-line model significantly reduced the duration of mechanical ventilation and weaning time in patients with COPD (all P <0.05), both the success rate of weaning and reperfusion at 48 h The incidence of pneumonia was no significant difference (all P> 0.05). Conclusion The Smartcare Intelligent Off-line Mode can shorten the mechanical ventilation time and weaning time in patients with COPD, which is an effective and safe weaning method.
