目的探讨“三四五”(三多、四有、五结合“的健康促进工作模式,三多是多方参与、多方投入、多种形式,构建工作立体框架;四有是有免费体检、有个体化干预、有典型推动、有全面普及,提供健康管理特色服务;五结合是与新农村建设、新型农村合作医疗、农村健康教育人员能力建设、农村重大疾病预防控制和创建中医示范县区相结合,建立长效机制。)健康促进模式在农村居民中的应用。方法对6676名农村居民进行问卷调查及体格检查,建立健康档案后提出个体化健康促进干预处方,并随访监督执行情况。结果干预后,农民的健康知识知晓率显著提高(P<0.05),不良生活方式明显改善(P<0.05),高血压和糖尿病管理依从性显著提高(P<0.05)。结论”三四五“健康促进模式对农村居民的健康状况改善具有显著的效果,值得推广。
