过去30多年里,关于血压与心血管事件之间的J型曲线一直都是备受关注的话题。众所周知,高血压是心血管疾病最重要的危险因素之一,积极有效的降压治疗可以显著降低心血管事件发生的风险。然而却有大量研究发现,舒张压的降低,也伴随着冠心病和其他不利后果的风险增高。这种效应在接受治疗的冠心病患者中更加显著。因此,强化降压的理念受到了巨大的冲击,而降压目标究竟该如何选择,再一次成为了内科医生们所面临的难题。对高血压中的J型曲线的回顾与分析会为我们制定科学合理的降压策略提供依据。
Over the past three decades, the J curve between blood pressure and cardiovascular events has been a topic of great concern. It is well-known that hypertension is one of the most important risk factors of cardiovascular diseases. Active and effective antihypertensive treatment can significantly reduce the risk of cardiovascular events. However, a large number of studies have found that the reduction of diastolic blood pressure is also associated with an increased risk of coronary heart disease and other adverse consequences. This effect is even more pronounced in patients with coronary heart disease undergoing treatment. Therefore, the concept of strengthening the antihypertensive has been a huge impact, and how to choose the goal of antihypertensive, once again become a physician facing problems. The review and analysis of J curve in hypertension will provide the basis for us to formulate a scientific and reasonable antihypertensive strategy.
