目的探讨巨细胞病毒(CMV)对儿童重型β-地中海贫血(地贫)儿童异基因造血干细胞移植(Allo-HSCT)后180 d外周血T细胞亚群的变化。方法对我院儿科造血干细胞移植中心2010年1月~2011年1月儿童重型地贫Allo-HSCT 35例病例半年后的T细胞亚群进行分析,比较有CMV(13例)和无CMV感染组(22例)在造血干细胞移植后早期T细胞的变化。结果半年内有CMV组CD8+比率高于无CMV感染组,但CD4+比率低。在CMV感染组伴随CD8+CD28+细胞比率降低,CD8+CD28-细胞比率和CD8+比率升高,同时发现CD8+与CD8+CD28-细胞比率呈线性相关性。结论重型β-地中海贫血儿童在异基因造血干细胞移植后,CMV感染后会导致CD8+CD28-细胞堆积,是构成CD8+细胞升高的主因,且两者比率呈线性相关。
Objective To investigate the changes of T lymphocyte subsets in peripheral blood of 180-day allogeneic hematopoietic stem cell transplantation (Allo-HSCT) in children with severe β-thalassemia (thalassemia) after cytomegalovirus (CMV). Methods T-cell subsets of 35 children with severe thalassemia Allo-HSCT in our hospital from January 2010 to January 2011 were analyzed after six months. There were 13 cases of CMV infection and no CMV infection (22 cases) in early stage after hematopoietic stem cell transplantation. Results Within six months, the ratio of CD8 + in CMV group was higher than that in CMV-free group, but the ratio of CD4 + was low. In the CMV infection group, the ratio of CD8 + CD28 + cells was decreased, the ratio of CD8 + CD28- cells and CD8 + cells increased, and the ratio of CD8 + to CD8 + CD28- cells was linearly correlated. CONCLUSION: After allogeneic hematopoietic stem cell transplantation in children with β-thalassemia major, CD8 + CD28- cells accumulate after CMV infection, which is the main cause of the increase of CD8 + cells, and the ratio is linearly correlated.
