对149例住院的2型糖尿病患者和37例健康体检者,使用三维实时超声仪测定心脏舒张和收缩状态时的具体数据,参照椭圆体的体积计算公式获取心脏在周期运动中的容积变化率,根据通常心脏彩超检查结果分为健康对照组、正常糖尿病组、心功能异常糖尿病组和形态学异常糖尿病组;和心电图检查结果分为健康对照组、心电图正常糖尿病组和心电图异常糖尿病组,比较各组间的心脏容积率,同时观察主要反映左心功能的EF值;结果:根据心脏彩超检查结果分组时,健康体检组的全心容积变化率和单位时间容积变化率分别是(4.17±2.10)%和(6.84±1.48)%,2型糖尿病心脏彩超正常组分别是(3.82±2.60)%和(5.58±1.12)%,2型糖尿病心功能异常组分别是(2.93±2.58)%和(4.89±1.07)%,2型糖尿病心脏形态学异常组分别是(2.66±2.29)%和(4.06±1.33)%,组间比较差异有统计学意义(均P<0.05);根据心电图检查结果分组可见EF值、全心容积变化率和单位时间容积变化率组间比较均有显著性差异(均P<0.05);结论:三维实时超声检查的心脏容积变化率可以用于糖尿病心脏病的早期诊断,全心容积变化率和单位时间全新容积变化率较EF值更敏感。
149 patients with type 2 diabetes mellitus and 37 healthy subjects were examined with 3D real-time system to measure the diastolic and systolic parameters. The volumetric change rate of the heart during cyclic exercise was obtained with reference to the volume calculation formula of the ellipsoid. According to the results of conventional echocardiography, the patients were divided into healthy control group, normal diabetic group, abnormal cardiac function diabetic group and morphologically abnormal diabetic group. The ECG results were divided into healthy control group, normal electrocardiogram diabetic group and abnormal electrocardiogram diabetic group Results: According to the results of echocardiography, the change rate of total heart volume and the volume rate of change per unit time were 4.17 ± 2.10, % And (6.84 ± 1.48)%, respectively, which were 3.82 ± 2.60% and 5.58 ± 1.12%, respectively in type 2 diabetes mellitus group, and 2.93 ± 2.58% and 4.89% in type 2 diabetic patients with cardiac dysfunction group, ± 1.07)%, respectively.
