目的观察幽门螺杆菌(H.pylori,Hp)感染对儿童身高生长的影响。方法福建医科大学附属第一医院儿科于2001年6月至2004年10月,采用整群抽样选择454例儿童进行血清Hp抗体检测、身高检查及社会经济状况和父母亲身高问卷调查。血清Hp抗体阳性者进行粪Hp抗原测定;血清Hp阴性者随访结束前再次检测血清Hp抗体。在研究开始、随访1年半及3年时分别进行1次体格生长指标测量及评价。结果60例(13·2%)儿童感染Hp,290例(63·9%)儿童未感染;Hp感染儿童3年身高增长值明显低于未感染儿童,调整后Hp感染和未感染儿童身高增长值及95%可信区间分别为21·4(20·6~22·3)cm、22·1(21·7~22·5)cm,两组校正均值比较,P=0·074。结论Hp感染与儿童体格生长有关联,主要体现在身高增长;Hp感染可能是影响儿童体格生长的环境因素之一,但影响的程度较弱;在社会经济状况相对不发达的地区,营养因素和遗传因素仍是儿童身高的主要影响因素。
Objective To observe the effect of Helicobacter pylori (Hp) infection on the height growth of children. Methods The pediatric department of First Affiliated Hospital of Fujian Medical University from June 2001 to October 2004, the use of cluster sampling 454 children were selected for serum Hp antibody test, height check and socio-economic status and parental height questionnaire. Serum Hp antibody positive stool Hp antigen determination; serum Hp negative again before the end of the test serum Hp antibody. At the beginning of the study, 1 year and a half years follow-up and 3 years respectively, a physical growth index measurement and evaluation. Results Hp was found in 60 (13.2%) children and 290 (63.9%) children were uninfected. The height growth of children with Hp infection at 3 years was significantly lower than that of non-infected children. The height growth of Hp infected and uninfected children was adjusted Values and 95% confidence intervals were 21.4 (20 · 6-22 · 3) cm and 22 · 1 (21 · 7-22 · 5) cm, respectively, and the adjusted mean of the two groups was P = 0 · 074. Conclusions Hp infection is associated with physical growth in children, mainly due to height growth. Hp infection may be one of the environmental factors affecting physical growth of children, but the degree of impact is weak. In areas where socio-economic status is relatively underdeveloped, nutritional factors and Genetic factors are still the main influencing factors of children’s height.
