目的::观察呼吸支持联合营养支持治疗在高龄重症肺炎患者中的应用价值。方法:临床纳入高龄(≥70岁)重症肺炎患者90例,根据患者治疗方案的不同分为研究组与对照组。研究组给予呼吸支持联合营养支持治疗,对照组仅基础呼吸支持治疗。观察两组患者呼吸衰竭纠正所需要的时间,治疗前后二氧化碳分压( PaCO2)、氧分压( PaO2)、体重、血红蛋白(Hb)、血清白蛋白(Alb)水平等。结果:研究组患者呼吸衰竭纠正所需要时间为(4.61±0.72) d,对照组为(5.93±1.04) d,差异有显著性(P0.05)。治疗后,研究组PaCO2、 PaO2、体重、 Hb、 Alb分别为(43.79±4.58) mmHg (5.84±0.61 kPa,1 kPa=7.5 mm-Hg)、(80.71±4.34) mmHg、(55.28±3.35) kg、(102.49±4.88) g/L、(33.21±3.03) g/L,对照组PaCO2、 PaO2、体重、 Hb、 Alb分别为(50.02±5.50) mmHg、(69.90±3.85) mmHg、(48.14±2.53) kg、(86.06±3.47) g/L、(25.97±2.75) g/L,差异均有显著性(P<0.05)。结论:呼吸支持联合营养支持治疗在高龄重症肺炎患者,能够迅速纠正患者呼吸衰竭情况,改善呼吸。此外,其还能够有效改善患者的营养状况,值得推广。“,”OBJECTIVE: To observe the application value of respiratory support combined with nutrition support therapy in elder-ly patients with severe pneumonia. METHODS: 90 elderly patients (≥70 years old) with severe pneumonia were divided into re-search group and control group according to different treatments. The research group received respiratory support combined with nutri-tion support therapy and the control group received respiratory support therapy. The required time of correcting respiratory failure and the levels of PaCO2 , PaO2 , weight, Hb and Alb of two groups before and after surgery were observed. RESULTS: The required time of correcting respiratory failure of research group was (4.61 ±0.72) d while the time of control group was (5.93 ±1.04) d (P0. 05). After surgery the levels of PaCO2 , PaO2 , weight, Hb and Alb of research group were (43. 79 ± 4. 58) mmHg, (80. 71 ± 4. 34) mmHg, (55. 28 ± 3. 35) kg, (102. 49 ± 4. 88) g/L and (33. 21 ± 3. 03) g/L. And the levels of control group were (50. 02 ± 5. 50) mm-Hg, ( 69. 90 ± 3. 85 ) mmHg, ( 48. 14 ± 2. 53 ) kg, ( 86. 06 ± 3. 47 ) g/L and ( 25. 97 ± 2. 75 ) g/L ( P<0. 05 ) . CONCLU-SION: For elderly patients with severe pneumonia, the respiratory support combined with nutrition support therapy can rapidly correct the respiratory failure and effectively improve the patients’ nutrition.
