目的 :研究人类龈沟液中性粒细胞弹力酶的动态活动方式和最高活性率 ,从而探讨其作为一项定量检测牙周状况指标的可能性。方法 :利用滤纸条法 ,从 33例慢性牙周炎患者提取 2 17份龈沟液样本 ,用中性粒细胞弹力酶的特异性底物对该酶在龈沟液中的活性进行动态定量测量 ,从而计算该酶最高活性率 (MR EA ,单位为每个牙部位mAbs·min- 1 ) ,并将结果与临床牙周状况进行综合比较分析。结果 :龈沟液中性粒细胞弹力酶可分为 5种不同的酶活动方式 (A ,B ,C ,D ,E) ,其相应的MR EA存在显著差异 (P <0 0 1)。在临床牙周状况相同的牙部位 ,弹力酶活动差异有显著性 ,各种临床部位的弹力酶活动方式高 (A % +B % ) 低 (D % +E % )分布情况如下 :健康部位0 % 92 % ,牙龈炎部位 6 % 72 %和牙周炎部位 2 1% 5 4%。健康部位的MR EA值≤ 3 0 ,而病变部位的MR EA值存在明显差异 ,具体分布如下 :75 %牙龈炎部位的MR EA≤ 3 0 ,13% >6 0 ,4% >10 0 ;而 6 0 %牙周炎部位的MR EA值≤ 3 0 ,30 % >6 0 ,2 1% >10 0。结论 :MR EA能作为定量测定龈沟液中性粒细胞弹力酶活动的适当指标 ,可以用来定量检测牙周状况。
OBJECTIVE: To study the dynamic activity and the highest activity of neutrophil elastase in human gingival crevicular fluid and to explore its potential as a quantitative indicator of periodontal status. METHODS: Twenty-two samples of gingival crevicular fluid from 33 patients with chronic periodontitis were extracted using the filter paper method. The activity of the enzyme in gingival crevicular fluid was dynamically quantified using a specific substrate for neutrophil elastase (MR EA, in units of mAbs · min-1 per tooth) was calculated, and the results were compared with the clinical periodontal status comprehensively. RESULTS: Gingival crevicular fluid neutrophil elastase was divided into five different enzyme activities (A, B, C, D, E), and the corresponding MR EA was significantly different (P <0.01). There were significant differences in the activity of elastase between the same periodontal conditions and the distributions of high (A% + B%) and low (D% + E%) elastin activity in various clinical sites were as follows: 0 % 92%, gingivitis site 6% 72% and periodontitis site 21% 54%. MRA values of healthy sites were ≤30, but MR EA values of lesion sites were significantly different. The specific distributions were as follows: MR EA ≤ 30, 13%> 60, 4%> 10 0 in 75% of gingivitis sites MRA values of 60% of periodontitis sites were ≤30, 30%> 60, 21%> 10. CONCLUSION: MR EA can be used as a suitable indicator for the quantitative determination of neutrophil elastase activity in gingival crevicular fluid and can be used to quantitatively detect periodontal status.
