2型糖尿病(T2DM)发病机制尚未完全阐明,一般认为主要与胰岛素抵抗(IR)和胰岛B细胞分泌缺陷有关,而IR是T2DM发生的基础,并存在于T2DM发生发展的全过程。IR的发生机制十分复杂,呈现出胰岛素抵抗明显的异质性。IR均与胰岛素的信号转导障碍以及胰岛素作用调控激素异常等环节的障碍有关,不仅存在于外周组织中,B细胞自身也存在IR,并参与T2DM发生发展。
The pathogenesis of type 2 diabetes mellitus (T2DM) has not been fully elucidated. It is generally believed that it is mainly associated with insulin resistance (IR) and pancreatic islet B cell secretory defects. IR is the basis of T2DM and exists in the whole process of T2DM. The mechanism of IR is very complex, showing obvious heterogeneity of insulin resistance. IR and insulin signal transduction disorders and insulin regulation of hormone abnormalities and other links of the disorder, not only in peripheral tissues, B cells themselves also exist IR, and participate in the development of T2DM.
