目的观察犬心跳骤停复苏后心肌细胞凋亡的变化.方法体外电击诱发犬室颤,3min后复苏,12只犬随机分为2组,CPR组,空白对照组,每组6只,采用Swan-Ganz漂浮导管监测复苏前和后6h的CO和PAWP,6h后取心肌组织,TUNEL法检测心肌组织凋亡的形态学改变.结果两组各血流动力学指标在心跳骤停前无统计学差异,CPR组的MAP在复苏成功即时高于空白对照组,随后开始下降,在复苏后4、6h低于空白对照组.CPR组的PAWP从心跳骤停前的(5.00±1.26)mmHg一直上升,到复苏后达高峰(28.83±4.79)mmHg,各观察点均高于空白对照组,CO在复苏成功后随时间延长而下降,6h降至最低,复苏后各观察点均低于空白对照组.CPR组心肌细胞凋亡指数高于空白对照组.结论电击诱发室颤犬复苏成功后存在着心功能不全和心肌细胞凋亡增加.
Objective To observe the changes of cardiomyocyte apoptosis during cardiac arrest and resuscitation in dogs.Methods In vitro shock induced canine ventricular fibrillation and resuscitation after 3 minutes, 12 dogs were randomly divided into two groups: CPR group and blank control group, -Ganz floating catheter to monitor the CO and PAWP before and 6 h after resuscitation, and after 6 h, the myocardial tissue was taken out and TUNEL method was used to detect the morphological changes of myocardial tissue.Results There was no statistical difference between the two groups in the hemodynamics before cardiac arrest Difference, MAP of CPR group was higher than that of blank control group immediately after resuscitation, and then began to decrease, which was lower than blank control group at 4,6h after resuscitation.The PAWP in CPR group increased from (5.00 ± 1.26) mmHg before CPR , Reaching the peak after the resuscitation (28.83 ± 4.79) mmHg, the observation points were higher than the blank control group, CO after the success of recovery decreased with time, decreased to a minimum 6h after resuscitation observation points were lower than the blank control group .The apoptosis index of cardiomyocytes in CPR group was higher than that in the blank control group.Conclusion There was heart failure and increased apoptosis of cardiomyocytes after the success of shock-induced canine dog resuscitation.
