目的:分析阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的血压节律变化情况。方法:选取重庆市长寿区人民医院收治的50例OSAHS患者作为观察组,另选取50例非OSAHS患者作为对照组,对所有入选者进行多导睡眠图(PSG)和24 h动态血压监测,分析两组患者血压变化情况及不同OSAHS分度患者之间的血压情况,对比不同血压类型患者其睡眠监测指标之间的差异。结果:重度OSAHS患者的白天、夜间平均血压、夜间平均最低血压均明显高于对照组,差异具有统计学意义(P<0.05)。比较各组之间的血压晨峰比例、晨起血压升高值、夜间血压下降率差异无统计学意义(P<0.05)。勺型血压患者血压晨峰比例为61.9%,明显高于非勺型血压患者,其最长呼吸暂停时间、平均呼吸暂停时间、平均低通气时间和氧减饱和指数明显低于勺型血压患者,其夜间最低血氧饱和度高于勺型血压患者,差异具有统计学意义(P<0.05)。结论:OSAHS会引起患者血压的非勺型变化,血压的非勺型变化与血压晨峰存在一定的相关性。
Objective: To analyze the changes of blood pressure rhythm in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). Methods: Fifty patients with OSAHS admitted to Changshou District People’s Hospital of Chongqing were selected as the observation group. Fifty non-OSAHS patients were selected as the control group. Polysomnography (PSG) and ambulatory blood pressure monitoring Two groups of patients with changes in blood pressure and OSAHS patients with different sub-levels of blood pressure, compared with different types of blood pressure in patients with sleep monitoring differences between the indicators. Results: The mean daytime blood pressure, nocturnal mean nighttime blood pressure and nocturnal mean blood pressure were significantly higher in patients with severe OSAHS than those in the control group (P <0.05). There was no significant difference in morning blood pressure ratio, morning blood pressure rise and nocturnal blood pressure decline rate between groups (P <0.05). The peak morning blood pressure of patients with spoon-type blood pressure was 61.9%, which was significantly higher than that of patients with non-spoon-type blood pressure. The longest apnea time, average apnea time, mean hypopnea time and oxygen desaturation index were significantly lower than those with spoon- The lowest nocturnal oxygen saturation in patients with spoon-shaped blood pressure, the difference was statistically significant (P <0.05). CONCLUSIONS: OSAHS can cause non-dipper changes in blood pressure. There is a certain correlation between blood pressure non-dipper changes and blood pressure morning peak.
